Ki-67

The ki-67 is the proliferation rate or how rapidly the cells are dividing and forming new cells. It does not determine staging. Staging is due to size of tumor and if it has spread to lymph nodes or into tier organs. My stage is 1 my ki-67 is 97% and it is high grade 3 . Together it forms a picture of what type of cells as far as aggressive and what treatments will be needed.

Just a heads up that Ki-67 is seldom used anymore because it's not an accurate test. What is 5% to one pathologist is 35% to another, there is no true standard. Some pathologist out there labeled me as having 35% yet I'm Luminal A and Low risk. My oncologist said to discount the ki #.
Here is a study showing how the ki %'s were all over the place on the same sample depending on pathologist - all from leading cancer clinics. It's the human factor that makes it inaccurate. http://connection.ebscohost.com/c/articles/79460867/how-reliable-ki-67-immunohistochemistry-grade-2-breast-carcinomas-qa-study-swiss-working-group-breast-gynecopathologists

This doesn't make any sense to me at all. Actually none of this does. I am new to this site, 37 years old diagnosed in June at age 36 with IDC. My stage is 1 at the moment, grade 2, my Ki-67 was less than 10%, my oncotype dx score is 25. My BS told me with that score there is a 16% chance of recurrence without chemo. I am confused and scared and am here for some support.

Hi Carjeanne3! You and I are similar in our cancers...I'm TN as well, Grade 3, ki67 is 95%. I finished Chemo on 1/12/17 and was doing really well until my 1 month follow up with my onc. She said to me "you are high risk of relapse due to your ki67 being so high." I've been freaked out ever since! Trying to get my mind set back to the positive, but has been hard. Every pain or twinge scares me. Since we are TN, I know 1 of the only things we can do to prevent recurrance is eating healthy and exercising. I've lost 18 lbs by eating better and exercising, but I still have a way to go. Hope things are going well for you. Take care!

my KI67 was less than 5% on the pathology report from my biopsy. But the pathology from my lumpectomy showed a mitotic rate of 2 or 3. I can't remember exactly - but I know it wasn't a 1. So those two numbers seem to contradict each other. I would have expected the mitotic rate to be 1 based on a KI-67 less than 5%.
My Oncotype turned out to be 11 - so no chemo was recommended. But that 2 or 3 mitotic rate still concerns me....
Anyone else have a difference like thatbetween their mitotic rate and KI-67?
