Does insurance cover nipple revision?

Seems to me that would be purely cosmetic, but thought I would ask. I am not happy with my results and was wondering. If not, I can't do it.

If you have breast cancer & have had mastectomy, I would believe nipple reconstruction would be covered under legal health laws.

I know nipple recon is initially covered, but I wasn't sure if it covered a second nipple surgery when everything else is okay...
