Lumpectomy area hardness after surgery and radiation

I am two weeks post radiation treatment. The site of my lumpectomy scar is very hard and when I was massaging it today with cream, a clear yellow liquid came out of my nipple. Should I be concerned? Has this happened to anyone else?
