Use a good voice recorder.

I've recorded almost everything Ive been to where there is a consultation type appt. It has been absolutely invaluable! In fact we wouldnt have been able to do without it. Of course it is VERY WISE to ask if they mind that you record the consultation/appt I think all but 2 were happy. Some Docs were totally intrigued and were going to make recommendations that patients do this. FOr the ones who are not happy I say OK, thats fine, but if theres anything important that I want to remember Im going to have to stop you and repeat what youve said myself. Because the public health system here can be so stretched at times and as such, in a hurry, they usually "come around" lol  ALso when they know how much there is to "take in" they are very good about it. There are differnt laws about audio recording in different countries, so this asking 1st would stop any extra dramas that you most certainly dont need to face.Oh and dont forget to have the recording going WHEN you ask as extra proof.

99.99999% of the time when I go for anything to do with medical, I just get brain fade. Ive got an expensive Olympus which I use in audio work as well as an mp3 player and other things.  B4 I bought this I tested out the little elcheapo mp3 players that said they do voice recording, and IMO they are pretty useless unless you are right up next to sound source and doctors can move around quite a bit depending on the consultation type. You dont need a really expensive one but Im sure you could get MUCH better than the next to useless elcheapos for a reasonable price.
