Finally!! After 3 months I have clean margins. I meet with my BS next week for my post op, however I have some questions about my path report, and wondering if you ladies can help me out. Also, I need to meet with the RAD ONC, and wondering what questions I should/need to ask.

1. My first pathology report says:

A. DCIS, Cribriofrm Type, Nuclear Grade 2 Measuring 2 mm in greatest dimension, associated with microcalcification,margin of resection negative for carcinoma.

- Intraductal Papilloma.

B,Additional excisional biopsy: DCIS present in 3 blocks Cribriofrm grade 2, approaching within 1mm of the resection margin.

-Intraductal Papilloma.

My question is, do they not add up how big the DCIS is if it's multi focal? My summery is 2mm part A and Multi focal part B.

Second Path report says: Residual Focus of DCIS present in four slices and extending into intraductal papilloma and present within half a mm of superior margin.

So, does this mean that all together I only had 4mm of DCIS ??

Third PA showed nothing... and margins are clear

I guess I'm confused about how much I had after reading report.

Oh gosh I wish I could help you but let me tell you once Beesie reads this she is the Queen of this website and with her knowledge I bet you she can decifer it for you.......I thought I was doing a great job and why would I need rads as I rated myself on the Van Nuys Chart as a 4 and when meeting with the radiologist oncologist I was told I was an 8......so as far as the pathology report goes.....I am of no help!!!!!

I'm not sure if they add it up, or how much you had - but I don't see where you're getting 4mm.  All I see is the 2mm measurement for Part A.  There's no measurement listed for Part B.

When you have a re-excision with more DCIS, getting a precise measurement of the original tumor becomes problematic.  The important points are that you have clear margins now, the DCIS that showed up on the second excision was no worse in grade or type than that from the first, and the tumor size was small.

You should ask the breast surgeon and rad onc to explain the pathology reports and how the three of them fit together.  S/he may be willing to estimate the size of the original tumor or may give you an answer similar to mine above.
