Study on religion and the patient-doctor relationship

My name is Jason Silverstein and I am a graduate student at Harvard Divinity School.  I am studying the role of religion and the patient-doctor relationship.  I want to better understand what role spirituality plays, and what role it should (or should not) play, in medical care.

I would like to invite you to participate in an online survey about religion and the patient-doctor relationship. 

There are 10 questions on the survey and I do not expect this survey to take more than 5-15 minutes to complete.   The survey is located at:

If you would like to participate, please read the following:  Your participation is voluntary and you may stop the survey at any time.  You may answer as few or as many questions as you would like.  There is no compensation.  The overall benefit is insight into spirituality and medicine.  Your answers are strictly confidential and remain anonymous.  No identifying information is collected as part of the survey.  Responses will be reported only in the aggregate, except for open responses, which may be quoted anonymously.  Responses will never be reported in such a way that a respondent's identity can be inferred.  This project is neither funded by outside sources nor does it have the official support of Harvard Divinity School.
