Need To Know Hormones to Check

Can somebody please tell me what specific hormones I need to have my doctor put on a prescription to check. Is estrogen sufficient, or does it need to say; Estrogen, Estradiol, Estrone? Testosterone; does it need to be Total, or Free and Total, or Bioavailable, Free and Total? I figured out the Vit D 25-Hydoxy okay. PTH-there are 5 different tests. I don't know which one I should ask for...Antibody, Biointact, C Terminal, Intact, PTH-Related Protein. I read somewhere about 17 Hydroxylase. Is that something related to the adrenal glands? What about the 17 Hydroxylase deficiency? Is that it, or is it 17 Hydroxyprogesterone Response? If anyone can give me advice on what tests they had done it would be very helpful to me. I doubt my doctor will even want to test me, but I need to have the information to take to another doctor I will have to find.
