Does Having IDC and DCIS change your treatment?

I was initially diagnosed with IDC and then when I saw the pathology report after surgery, it said 80% IDC and 20% DCIS.    Would the fact that I also have DCIS change the treatment that I would get if I just had IDC - i.e., would the recommended treatment be less aggressive?  Thanks.

No such luck. 

The treatment is based on the most aggressive component of the tumor, which in your case would be the IDC.  I guess maybe you're thinking the IDC part was only 80% of the "2.5 cm", so if the malignant part of the tumor was smaller than they thought, the treatment could be less aggressive...  I don't know for sure, but I doubt that would change anything.

I believe as Otter says, that the treatment goes accordingly to your most aggressive invasion portion.  Since your tumor is considered stage II I'd think that would mean, that your IDC was at least 2+cm with 20% or the other remaining was DCIS.  They'll treat as to the stage II, and the DCIS won't even be considered really.

I had both IDC with DCIS and another primary that was ILC with LCIS.  They treated for the IDC and do not even combine the 2 primarys in so far as treatments go even though the two together were larger than your one, they treated it as a stage one cancer, b/c the larger was less was than 2 cm. I hope this all makes sense.

Ductal cancer always starts in the duct - that's DCIS (Ductal Cancer In Situ) - In  Situ = in place, i.e. in the duct where it started. When it leaves the duct, it is IDC (Invasive Ductal Cancer) so everyone with IDC would have DCIS.

The treatment will be based on the characteristics of the IDC. For example, my lump was part DCIS and part IDC. I'm treated based on the 0.8 cm IDC.

I had a 10 cm DCIS tumor in my right breast, but I was treated aggressively for the smaller IDC. With further testing DCIS was also found in my left breast. I was already schueduled for a right mastectomy based on the size of the DCIS tumor and the IDC. I could have chosen a lumpectomy on the left, but because this meant I had two primary cancers and I was HER2+, I went with a bilateral mastectomy, chemo and Herceptin to increase my chances of not having a recurrence.

I don't regret the aggressive treatment, although I do still miss my breasts.

Thanks - no, I don't have a treatment plan yet.  I am waiting on the oncotype results to come back - such a looooong wait.  I also requested and took the CYP26 test.

For me, the DCIS was more of a surgical problem and the IDC a "treatment" problem. The IDC meant "chemo" but if I hadn't had the DCIS, I might've been able to have a lumpectomy instead of a mast. I don't know how widespread your DCIS is, but I think the answer to that will be important in determining any surgery. When/if they do node removal, that will also help determine treatment. Age also plays a part... the younger you are, the more likely you'll get the book thrown at you! xoxo Erica
