SIL running out of options/how to help?

Hi.  I'm new here.  My SIL is battling recurrent breast cancer.  She will be 39 in two weeks and has two children, 8 and 5.  She is my husband's only sibling and lives 15 hours away from us.

She has had difficulty tolerating every chemo she has been given.  The last round of chemo put her in the hospital with non-stop vomiting and diarrhea.  She has mets to the liver and lung.  The ones in the liver are growing, totalling over 15 cm worth of tumors.  The one in her lung is about 8 cm and pressing on her bronchial tube.  The chemo she has been on isn't stopping the growth.  They've tried radiation too.  Nothing is working.  She has fluid on her stomach that has to be drained today.  (Is that what is called ascites?)  The oncologist told her that they are running out of options.

I went up and stayed and helped in May.  My husband went up in June and did the same.  We are at a loss as to what to do.  How have you all helped from far away?  I would appreciate any suggestions.  

Also, any thoughts on what to get her for what will probably be her last birthday???
