My core needle biopsy report came back "stromal sclerosis with focal adenosis". My surgeon wants to see me next wk. I read on the website radiographics.org that "Sclerosing adenosis may present a diagnostic dilemma for the pathologist because stromal sclerosis and elastosis may mimic infiltrating carcinoma (3). Tubular carcinoma in particular presents a difficulty in differential diagnosis."

I am wondering if I should just have both "nodules" removed? Strong family history and previous biopsy. Any help and or knowledge is appreciated.

    Kimber,

Sometimes it is more difficult to distinguish pathology on biopsy. I think your surgeon is being very proactive (which this board favors) by having you come in review his or her recommendations.

Perhaps wide excision will be needed in your situation. If it remains not clear cut, a second pathology opinion is easily obtained from any of the prominent Comprehensive Cancer Centers around the country and other laboratories known in pathology. And tissue could also be obtained and held for the newer IHC (immunohistochemical) markers and gene arrays which are so rapidly expanding in use.

You sound like you're on the right path with your thinking.

Please let us know how you do, should you wish, and we'll be thinking of you.

    Tender
