Mastectomy after lumpectomy

Brief history. Dx Aug 06 at 40yrs. Stage 1 grade 2 Her2 elevated. Had lumpectomy, reexcision for clear margins, 33 rads, hyst/ooph and now I'm on Femara.
At my last appointment with my oncologist I showed her a small lump in my scar tissue. She said not to worry but after she felt it, she said we should take it out since it so close to the skin.
I'm not sure if I want to do this again or just go for the mastectomy (double).
What do follow ups consist of with a mast? Are you on a 6 month follow up? Obviously no mammograms. Do you get a yearly mri? I think it would be worth it not to have as much worry hanging over me. I'm just really scared to bring it up to my dr and equally scared to have surgery.
