2. How long for Initial healing? I went back to work at five weeks, but had complications that started later, due to my drains being removed too soon.
How long has it Been total? 9 mos.
3. Was the Pain More Or Less then Expected? Less, I was off pain meds within 3-4 days.
4. Any Complications? Yes What? Nearly lost SIEA flap 7 days post-op, but thank the Lord it recovered. Pseudobursa requiring re-opening of abdominal incision.
5. Did your Surgeon Estimate your finished Size before surgery? No And are you about that size now? I asked him to make the flaps as large as possible due to my intention to lose a lot of weight post-op. I didn't want to lose fat in the breasts and end up with nothing after everything I was going to go through. He did.
6. Before surgery Size & After surgery size (cup) Before, large B/small C. After, full C.
7. Did you have Nipples done? I will, in three weeks. What kind? Origami, from breast skin.
8. How did you like your Stomach Before Surgery? Hated it. How did you like it about 6 months after... HUGE improvement. I have only one stretch mark above my incision, which was previously above my belly button.
How much do you like it now? It's still pretty swollen, so I don't think I'll see the final result for a while.
9. Did you sleep in your bed, Hospital bed Or Recliner? Recliner for a month, then the sofa for a month.
Anything Else you would like to Add? Abdominal incision (re-opened in 11/07) is still very tight and I'm still experiencing a lot of swelling. No regrets, even though my doctors recommended lumpectomy and radiation. I didn't need chemo or rads with this option. I'm extremely happy overall, and cannot wait to finish!!

2. How long for Initial healing? SURGERY WAS 12/11. BREAST WOUNDS HEALED IN ABOUT A MONTH, BUT THERE IS SOME FAT NECROSIS. SOME IS DISIPATING, SOME WILL HAVE TO BE REMOVED STAGE 2. BECAUSE I WILL HAVE RAD ON CA SIDE, EXTRA SKIN/FAT LEFT THERE BECAUSE OF POSSIBLE DAMAGE. I GET SOME PAIN ON THAT SIDE, BUT MOSTLY BECAUSE ELASTIC FROM BRAS OR CAMIS DIGS IN.
HAD SKIN NECROSIS IN BELLY AND THAT TOOK ALMOST 4 MONTHS TO HEAL COMPLETELY... HAD TO HAVE IT DEBRIDED, HAD WOUND VAC AND THEN WET TO DRY DRESSING CHANGES. WILL STATE THAT I AM OBESE AND THIS WAS MOST LIKELY REASON FOR THIE PROBLEM. LACK OF BLOOD FLOW TO WOUND. BUT CONSIDERING I WAS CUT HIP TO HIP AND CAME OUT OF SURGERY ABOUT 15 LBS LIGHTER, MY NECROSIS WAS ONLY 21cm LONG AT ITS' WORST POINT.
3. Was the Pain More Or Less then Expected? NEVER HAD MAJOR SURGERY BEFORE. MOST OF MY IMMEDIATE PAIN AFTER SURGERY WAS MY ELBOWS FROM BEING HYPER-EXTENDED DURING 9 HOURS OF SURGERY. REALLY NO SEVERE PAIN, BUT ACHINESS IN BREASTS AND BELLY WAS PHYSICALLY TIRING.. AXILLA NUMB FROM SB, TINGLING/ITCHING IN BREASTS AND MORE THAN ANYTHING MY BELLY FELT 'SUNBURNT' FOR ABOUT 2 MONTHS... SKIN SURFACE VERY SENSATIVE, BUT AGAIN, NOT TRULY PAINFUL.
4. Any Complications? SEE ABOVE RE BELLY WOUND What? ONE THING THAT WAS/IS A CONCERN. BECAUSE PS WAS WORRIED ABOUT MESH INFECTING AND ME HAVING TO HAVE ANOTHER SX IF THAT HAPPENNED, COULD NOT START CHEMO LIKE WE HAD PLANNED. I'M 16 WEEKS OUT AND WON'T BE STARTING UNTIL NEXT WEEK BECAUSE PS DIDN'T CLEAR ME UNTIL WOULD WAS COMPLETELY CLOSED 2 WEEKS AGO, AND IT TOOK THAT LONGO FOR ONCO TO BE ABLE TO MEET WITH ME. SO LITTLE WORRIED ABOUT WHAT NASTY LITTLE CELLS MAY HAVE BEEN PARTYING IN MY BODY WHILE I WAS HEALING.

2. How long for Initial healing? Stage 1 surgery end of October returned to work (desk job) at 5 wks. Stage 2 beginning of March I returned to work 2 wks. after surgery. I was very fortunate that I never had radiation, so have had no healing problems.
3. Was the Pain More Or Less then Expected? Not much pain, only used the morphine pump a few times the first night after Stage 1 surgery. Only needed Extra strength Tylenol after hospital. Did not need any pain medication after Stage 2.
4. Any Complications? No
5. Did your Surgeon Estimate your finished Size before surgery? Yes, B or C. And are you about that size now? I am a C.
6. Before surgery Size & After surgery size (cup) see above
7. Did you have Nipples done? Yes, star flap created with breast tissue, not a graft.
8. How did you like your Stomach Before Surgery? How did you like it about 6 months after...
How much do you like it now? Stomach is definitely flatter now, and since stage 2 feels much better. The surgeon revised the suture line slightly and removed some fat necrosis. He also plicated (tightened) the lower stomach muscles.

Anything Else you would like to Add? I still think of DIEP surgery as a miraculous procedure. I was very fortunate and had a good outcome. My breasts are soft and natural. -

How long of a Delay if delayed? 4 months (they thought I was going to need rads...so that is why..but I didn't)
2. How long for Initial healing? How long has it Been total? I healed really well, no swelling..drains out on day 10! Felt pretty good after 2 weeks..back to work after 4 wks.
3. Was the Pain More Or Less then Expected? Pain was less than what I prepared myself for. Being unable to do things was the hardest part for me...it was depressing.
4. Any Complications? NO What?
5. Did your Surgeon Estimate your finished Size before surgery? Yes..he reduced my other side to a full C (I was a full D) and matched the reconstructed side. And are you about that size now? Perfect full C.
6. Before surgery Size & After surgery size (cup) full D to a full C...I love it!! I fit into clothes better!
7. Did you have Nipples done? What kind? Since my surgery was just Feb. 4 2008, my nipples haven't been done yet. I go for my next consult in early May.
8. How did you like your Stomach Before Surgery? I used to like it..but chemo made me gain some weight..and it was all there. How did you like it about 6 months after... I'ts only been 2 months out for me..and I love my stomach...he had to make a new belly button for me (it looks a little weird), but my stomach is as flat as a board!! He said he tried to save my belly button, but I was too thin..haha...that was nice to hear.
How much do you like it now? same...looking forward to scar reducing and seeing how it looks 6 months to a year down the road.
9. Did you sleep in your bed, Hospital bed Or Recliner? I slept in an electric recliner for 10 days..then my bed with pillows for a few weeks.

Anything Else you would like to Add? I will know more later how I like my new breast..but I am starting to feel better about it. It's not what you once knew! I hate the scar on it...and I worry about wearing a bathing suit and if it will show. Mine is too early to determine all that yet. I kept asking myself after the surgery...WHY DID I PUT MYSELF THROUGH THIS?? I think that is normal...because it is a BIG surgery with a long recovery...but now looking back...almost 9 wks out....IT WAS ALL WORTH IT!! I feel whole again!
Oh yeah...since I knew I wanted reconstruction..when I had my mast...my bs did a skin sparing mast. So when my breast was reconstructed...there was extra skin for the ps to use ...it looks much better..and I can wear low cut shirts without revealing scars.

I'm still healing. It's been 11 weeks and things are looking and feeling great. I won't have the stage 2 until late June.

MUCH more pain. The first week was AWFUL but 2 weeks out there wasn't much pain at all.
4. Any Complications? What?
Both breasts got infected and I had to have a second surgery one week after the first. I felt a 1000% better once the infected tissue was removed.
5. Did your Surgeon Estimate your finished Size before surgery? And are you about that size now?
I was a DDD pre-surgery and my PS estimated a C, which is where I am now. I want to go down to a small C or big B.

I had a big tummy before and lots of stretch marks. It looks and feels awesome today. There are a few spots that need to be lipo'd out, but I'd be happy even if nothing else happened.

I slept in a recliner for the first month. I tried to sleep in bed at 3 weeks and it was still too much.

I had a very rough surgery and recovery. I wasn't prepared for so much pain. I had no idea! However, with all that I would do it again in a heartbeat.

Dejaboo, I am paying close attention to this thread, thanks so much for starting it. I have big decisions to make as well.
Yummy - your plastic surgeon was ok with performing the TRAM even with the knowledge that radiation would be needed later? Or was that a decision that was made post-surgery? I saw a ps last week who won't do any sort of immediate reconstruction if radiation is going to follow.

M - I am at Dana Farber. I liked the plastic surgeon I met with last week (affiliated with Brigham and Women's) but I may get a 2nd opinion from a plastic surgeon at BI. We are lucky to have so many great options nearby.
Did anyone get either a TRAM or DIEP prior to radiation? That is my big question since I really, really want to have immediate reconstruction. Thanks!

I won't add answers about my DIEP (all went well without complications), but I will say my PS told my husband he had two choices during my 5+ hour surgery:
1. Sit in waiting room and go nuts, or
2. Go to mall, or find a bookstore and come back in a few hours.
My husband opted for #2, and was grateful for the suggestion as he fully intended to sit in the waiting room and probably go nuts! He was back in plenty of time, and I knew nothing about his absence until he told me. Good for all of us!

Well, Im lost right now. Just meet with the PS. I had my whole 'Plan' centered around doing a DIEP (I knew I had to be a Candidate) But I thought I would be. I sure have enough stomach to grab with both hands. He said I could get one C cup total...I asked him if he'd put it right in the center. I dont think he has heard that before!
Im so upset.
My choices now are Implants...Or a Latissimus Dorsi with Implants.
So I guess I will post my questions for that now...I dodnt think there are very many out there to reply though.

My husband tried to do some work on his laptop in the waiting room until the BS came out with the surgery report and SNB news. Then he went to lunch with my son and a friend showed up there, too, and then went to a movie with my son and got back to the hospital just minutes before the PS came out with the report. Part of me was irritated that he didn't suffer in the waiting room but the more rational part was happy that they were able to distract themselves from what was going on for over 8 hours. The good news was that they saw a movie I had dreaded being dragged to see. That helped to make it alright, too!
Pam, don't be afraid of implants. Doing it as a one-step procedure is, well, not exactly a piece of cake, but quick and cheaper than the other surgeries with great results.

Pam, There are plenty of implant chicks on this board. I have to admit that many who helped me (and apparently aren't having any problems) have moved on and don't 'do' the board anymore. I responded to your lat flap/implant post. Best wishes

This is a great thread. Everyone's experiences are so different I find it truly amazing.
Have to say I'm only just over the surgery and nowhere near finished but I'll fill in whatever blanks I can.
1. What kind did you have? Unilateral
Delayed Or Immediate? Well, I had immediate expander last March, then implant in August, but had a free TRAM on March 27 this year because I was not satisfied with the implant.
2. How long for Initial healing? How long has it Been total?
3. Was the Pain More Or Less then Expected?
Zero pain in chest or abdomen, not one hit of pain meds, not even Tylenol.
4. Any Complications? What? Hematoma that had to be surgically evacuated on hospital day 4. My flap was in very precarious position and its survival was in doubt so they wanted to do everything they could. Luckily, it's now been two weeks and all seems healing nicely except the whole foob is very hard. Color and temp good with good arterial and venous doppler signals.
5. Did your Surgeon Estimate your finished Size before surgery? And are you about that size now? My foob is huge, partly because of the swelling and hardness. Much will have to be remove at stage 2.

How much do you like it now? My tummy was a bit poochy, not bad. Just enough for one smallish foob.
Since I have not had any swelling to speak of at the stomach incision site, I LOVE LOVE LOVE my flat belly!
9. Did you sleep in your bed, Hospital bed Or Recliner? I was in the hospital for seven day, what with the two surgeries and the dicey situation, but was sleeping almost flat from day 2. Very little pulling. Once I got home, I tried the bed wedge pillow I had bought in preparation but found it uncomfortable and just went to two down pillows under my head the second night home, then down to one pillow. I sleep on my back with brief turns to my good side. I'm very comfortable.

11. Did you have any Complications? What? I did lose a lot of blood but they wanted to avoid transfusing me. I've been okay and have ordered some of my favorite iron compound preparation, Floradix, to help build myself up. The Floradix is great because it does NOT CONSTIPATE like iron tabs (Feosol, etc.)
Anything Else you would like to Add? My surgery was estimated to be about 8 hours but it turned into 11. I had sent my husband home for the duration and he was getting panicky. Finally, he got the call. He did drive in to see me in the PAC U, where I spent two nights, but it was a waste as I was not seeing straight, if you know what I mean. LOL Lovely experience. Felt nauseated for almost two whole days and could not drink anything except water. FWIW, make sure you will be able to get drinks that are not sweet because if you are like me, anything sweet on a sick stomach will turn it even more. I found plain seltzer water to be perfect.

2. How long for Initial healing? How long has it Been total? I am 4 months out from stage 1. I went back to work 6 weeks after surgery. The first 2 weeks were rough, but got better slowly after that.
3. Was the Pain More Or Less then Expected? About what I expected after reading on this board. The 1st 2 days were awful and more than I thought. PS said I would start to feel better on day 3, which I did.
4. Any Complications? What? I didn't have any complications. I did have a severe headache that kept me nauseous for 2 days and could not eat. They finally gave me something more than tylenol and I was finally able to eat 2.5 days after surgery.
5. Did your Surgeon Estimate your finished Size before surgery? And are you about that size now? Yes, he said a large B cup. I am probably a small C, so it's close. I'm happy about the size.
6. Before surgery Size & After surgery size (cup) C before, small C after.
7. Did you have Nipples done? What kind? I have not had stage 2 yet.
8. How did you like your Stomach Before Surgery? How did you like it about 6 months after...
How much do you like it now? I am thrilled with the tummy tuck. My PS will be doing lipo on my stomach and hips in stage 2, so I'm expecting it to look even better.
9. Did you sleep in your bed, Hospital bed Or Recliner? Rented a hospital bed and am so happy I did. Only cost $20 per month with insurance.

I am having my TRAM in less than two weeks and I am trying to read through everyone's comments. I have a question about number 9 - is the recliner or hospital bed more comfortable once you are home? Do you need it because the position is easier to sleep in or because it is easier to get out of bed/chair if it reclines up/down? I have an adjustable type bed - not really a hospital bed - in one of my kid's rooms and I was debating whether to swap beds with her or buy a new recliner since I don't have one. Has the recliner been more comfortable later on in the recovery?

I rented a hospital bed, but I think the adjustable bed would do the same thing. It was definitely comfortable to sleep/be in. I really liked that I could adjust it more as time went by where I could lie almost flat, but not quite. I am really happy I had the hospital bed and would do it again. I don't have a recliner so can't comment.

I recently heard a suggestion to use a chaise lounge - the backyard type - if you don't have a recliner. Especially the soft kind where the recliner is 'activated' by using your feet and no arms or tummy. It's cheaper and you can use it this summer to relax in outside. It's a thought.
I used a recliner and found it quite comfortable with a fitted sheet on it and all my blankets and stuff. But it is lever-activated and the lever was on the wrong side so that was a major hassle. I actually had to learn to climb out and back in with the footrest up. That was a sight, I'm sure.

I hope you're consulting with other PS's. Different docs will have different opinions and expertise. See at least two or three before you give up. Look for someone who specializes in DIEP, even if you have to go to another city. A lot of us have travelled for our surgeries, and for me it was well worth the effort.

Thanks, I will probably get a 2nd & 3rd Opinion. Although this surgeon Specializes in DIEP. He showed me how much of a the Tummy Is used...So I can kind of see where I dont have enough for 2 Cs

As far as ideas for family members is concerned -- my elderly mother was waiting for me post bil ped TRAM. I explained the situation to my surgeons. They stayed in contact with her by phone so she could wait in the motel near the hospital. It made it much easier for her.

I haven't had my surgery yet...4 days to go...Tuesday April 15 bilateral DIEP.
To answer the question concerning loved ones...I told my husband to go to work. This way he can keep busy instead of worrying. I have asked the nurse to call him about 1 hour before he can see me. This will give him time to shower and get to the hospital. No reason to hang out for hours doing nothing but wait and worry
. I would much rather he use, what will be holiday days, at home when I will need him most. While in hospital the nurses and my doctor will take good care of me I'm sure.
As for other loved ones (except my children) I have asked them to come to the hospital no earlier than day 3 (Day 1 surgery, Day 2 recovering, Day 3 come and visit) or just wait till I get home where all can be much more comfortable. I won't charge them exuberant amounts for parking and coffee either.

There was no keeping my family away for my surgery. Husband, mother, sisters and brothers all sat in the waiting room all day. I guess they entertained each other. Once the ps started my DIEP they didn't hear anything until it was over. My husband was furious and I'm told he let the doctor know. That was our first experience where no one communicated occasionally with the family about how things were going.
The ps had told me that it may be 8 hours, but I never dreamed that my family wouldn't get any information the whole time.

Hi Dejaboo - I hope you WILL get a second opinion about the Diep, it has to be so hard for you to switch gears and get info on the other procedures!
For what it's worth at this point in time, here's my answers to your original questions:
1. What kind did you have?
Unilateral Or Bilateral? Mine was unilateral (right side.)
Delayed Or Immediate? Immediate.
How long of a Delay if delayed?
2. How long for Initial healing? Still healing, 6 weeks later. How long has it Been total? I'm still not totally healed - my surgery took place on March 3, 2008 - my stomach incision is still open and draining, unfortunately.
3. Was the Pain More Or Less then Expected? MUCH more! I had 3 c-sections (last one 15 years ago) and went in thinking that the "tummy-tuck" portion of the Diep would be easier - I was soooo wrong!
4. Any Complications? Yes. What? Abdominal staph infection and gallbladder attack - then later, my abdominal incision opened. Also, swelling in my Diep breast - and cording up my right arm, preventing full motion.
5. Did your Surgeon Estimate your finished Size before surgery? We discussed it and I told him no later than a B cup. And are you about that size now? No - it's much larger than I expected - not in the front but on the side under my arm. I'm told it's swelling and it'll go down, but somehow I doubt it. Hopefully by Stage 2, with nipple reconstruction, the PS will be able to remove some of the extra fat.

7. Did you have Nipples done? Not yet.
8. How did you like your Stomach Before Surgery? It was ok, but too soft and fat, although I never had any stretch marks. How did you like it about 6 months after...I'm only 6 weeks finished, so I can't comment......my abdominal incision is still open and oozing, very gross to look at! My stomach muscles still spasm and I have a lot of pain if I stand or walk too much.
How much do you like it now?
9. Did you sleep in your bed, Hospital bed Or Recliner?
Added Questions: Recliner for the first two weeks.....my own bed with a lot of pillows after that.
10. How Long Was Your Hospital Stay? My initial stay was 5 days - readmitted to ER for staph infection, stayed 11 days.
11. Did you have any Complications? What? Staph infection and gallbladder problems (latter still not resolved.) Also I needed two blood transfusions in the hospital probably due to the antibiotics doing a number on my red blood cell count.
Anything Else you would like to Add? Leave yourself a LOT of time to recuperate from any TRAM flap surgeries......the risk of infection is much higher than other surgeries because of the time under anesthesia (I was in surgery for 7 hours.) You'll need a LOT of help once you get home - you need to keep your hands scrupulously clean while "milking" your drains.......it's recommended to shower two days in a row before your surgery with Dial antibacterial soap (not that it prevented MY staph infection!) Also, discuss with your surgeon how soon after surgery you can start exercising your arms to reduce the risk of scar tissue and cording. Good luck and God bless!

1. What kind did you have? Bilateral
Delayed on cancer side. Immediate on prophylactic side. They removed implant on cancer side that was placed in 2005.
2. How long for Initial healing? 6 weeks to even stand straight How long has it Been total? 7 months
3. Was the Pain More Or Less then Expected? Much worse than I was prepared for or really could have imagined I think.
4. Any Complications? Pain meds (I can do morphine only because of drug allergies) caused my respirations and oxygen saturation to get dangerously low. The answer was to stop the pain meds. I don't know if I can even adequately describe what the first 4 days post surgery were like. So not really any problems with the surgery, just after care. Turned out to be a communication issue with the resident.

7. Did you have Nipples done? Not this time. Had a nipple done in 2005 using the groin transplant method. That's what my PS wanted to do again this time and I declined.
8. How did you like your Stomach Before Surgery? Well, being 40 and 2.5 years of tamoxifen had given me a pooch. I was glad to see it go! How did you like it about 6 months after... The stomach itself is FLAT - it looks great from the side! However, the loss of stomach fat has accentuated the back, upper hip fat and that is my new area of obsession. Ha, ha!!
How much do you like it now? Like the way it looks. Can't stand the way it feels.
9. Did you sleep in your bed, Hospital bed Or Recliner? Tried the bed. It was a no-go. Tried the recliner and it was OK for a couple of hours. We ended up renting a hospital bed and I slept better than the other two places. I was just extremely uncomfortable no matter where I was. Even with the Ambien I would still wake up about every 45 minutes.

Anything Else you would like to Add? I think I'm probably not the "norm" and many of my issues were due to my size pre-surgery (small) and the amount of tissue and skin that were removed from my abdomen in order for my PS to perform bi-lateral. He has since told me that mine was his tightest and most difficult closure ever and that they had to flex my body "way past 90 degrees" in order to get me closed. I'm 7 months post surgery and by the end of the day, when my body is fatigued, it is STILL difficult for me to stay fully upright because my skin is SO TIGHT. And it just feels weird. If I lay flat on the floor well, a) it's still hard for me to do; and b) you could bounce quarters off my stomach. So, I think many of the issues I have probably originate from this.
My surgery was 16 hours long but my husband received updates by phone from the OR about every 2 hours. He said they consisted of "everything is going good. She's doing good. We'll call again in 2 hours." You all have had good suggestions about alternate things for family to do. They took me back at 6:30 am. Surgery started approximately 30 minutes later. My husband said he did pretty good until about 7:00pm that night. Then he said he just fell apart and spent much of the next 4 hours standing outside the hospital, bent over double, crying. I'm sure he would have been much happier at the Bass Pro Shop which is a 20 minute drive from the hospital!!! And I would have been none the wiser!

Wow Abby............you've been through so much! I know how you feel about your tight stomach - mine is that way, and it has muscle spasms, but I'm not exactly a "thin" person, so I'm hoping it'll eventually stretch out. So many ladies have said things really improve after 1 year, so I'll be praying that'll be YOUR story in another 5 months.

LizInFl - I had my tram on 4/2 and have been sleeping in my adjustable bed since I got home. It's been wonderful to be able to adjust it to exactly the comfortable position I need. About a week after my surgery I started experiencing swelling in my feet and legs. The bed has been a great help in being able to raise my feet up to reduce the swelling.
I have recliners on my sofas and have used them some but found them harder to get in and out of than the bed.
