Radiation on silicone implant?
Just as I was finishing a tattoo touch up and moving to every 6 months appointments, I found a lump in my reconstructed breast - IDC again!!
I did not need chemo or rads with my first dx but with this recurrence I'm getting ready to start chemo and then will have radiation to my reconstructed breast that has a silicone gel implant.
Anyone have this? I am wondering about the problems and cosmetic outcome.
Have a great day everyone!!

Hi Dawn,
I wish I had an answer for you.
I just had my bilateral with tissue expanders for my re-diagnosis, so I don't know if they plan on radiating me before I have the exchange or not.
I do have a friend who had rads after implant and she was ok. But I don't know all the specifics.
So sorry you have to be "here" again. I know it really sucks.
Love,

Thanks for responding g!
I can't seem to find much info about my situation. I got a reply from bethNY on the reconstruction thread - she said the implant would need replacing a few months after rads - UGH. Makes sense of course but my first thought was - MORE SURGERY - YUCK!! Oh well.
I see my oncologist Monday morning to go over chemo and set that up. Got my past shoulder length hair cut off today - it's been YEARS since my hair has been this short!!
Hope you are having a good weekend and let me know how you are doing with your re-diagnosis too!

Dawn,
I have saline implants and they are fine over a year after rads. Hope you do well!
g -
LittleG - so you had your saline implants and then had radiation with them in or did you had expanders in during radiation?
Sorry for being nosey.................
Dawn -
I had cosmetic implants for about 7 years now. Then dx with bc, then lumpectomy and then rads. My implants are still in tact, and no damage from the rads. And you're not being nosey :-)

Dear Dawn,
What did your radiation oncologist say about radiating through the silicone? Saline radiates pretty much the same as breast tissue. (I had a saline implant that was radiated.) I know that my radiation oncologist wanted to be sure that the dosage of radiation to the chest wall behind the implant was uniform. I think that your doctor will have some opinions on this.
Best of luck with it all.

Hi Dawn,
I just finished 4 months of chemo (Andriomycin/Cytoxin & Taxol) on May 4th and just had a dbl mastect w/ silicone implants w/ Alloderm skin grafts 10 days ago. I start 7 weeks of radiation soon. My entire team work together for optimal results and this is what I have been told: conservative radiologist typically do not like to radiate around silicone as it is diffucult but not impossible. It just makes their job a little more difficult which is why the "mapping" portion before rad trmt is crucial. My plastic surgeon in Boston is phenom and I have been told that there will be skin shrinkage from the radiation which may cause the breast to "lift". If the end result is unacceptable to you, it will require additional surgery. -
Beverly Joyce,
Just curious--who is your Boston PS? I always want to know who the phenoms are in my area! -
thanks Littleg for your response - hope you are doing well with your expanders (drove me crazy w/one!!!)
Hope M. - I can't for the life of me remember exactly what my radiation onc said about the silicone - I will question her again of course! Must not have been a problem or it would have stayed in my head!!! well, maybe...........
beverlyjoyce46 - sure hope you are feeling well after your surgery! I start chemo on 6/1 - with rads starting after. Guessing that time will tell and that I will just have to have patience!!!
Thanks!

Hi Eveybody,
I just logged on for the first time. I had a bilateral mastectomy with immediate reconstruction with silicone implants three weeks ago and will be starting chemo soon. I also just found out that I will require radiation since there is evidence of cancer cells in my lymphatic vessels. We weren't expecting this so I am concerned about what will happen to the implants after. I REALLY don't want anymore surgery!! I had a blood clot burst in my chest ten days after my mastectomy which required emergency surgery where they removed the implant and had to put it back in after....soooo done with surgery...... Anybody have any other radiation with silicone stories???? SUZ

Hello Suz,
One year ago, I had bilat mastec with immediate reconstruction with expanders/alloderm grafts. 6 months later, the expanders were replaced with silicone implants - great results and they looked so real. Within a year of initial diagnosis, I had recurrence to left side and had to undergo recent radiation. Per advisement from surgical onc, rad onc, med onc and plastic surgeon, I had to remove my silicone implant prior to radiation to get the best treatment field.
Now I have a concaved left side and a great looking right side.My ps is going to evaluate me to determine if I can re-expand again instead of major surgery involving a lat flap. She said the expander/silicone implant would be smaller due to the damage to radiated skin and she would later match the right side. Plus she forewarned me the expander might fail- but I would have choices for other reconstruction.
I'll be thinking of you.

I started this thread way back in May and I've just finished 4 rounds of A/C and 12 weekly taxol/herceptin - I meet with my radiation oncologist day after tomorrow for a re-consult and ct scan, markings etc to start radiation in about two weeks.
Guess I'll be answering my own questions soon and I'll certainly share!!

I had radiation to silicone implants and had no problems. I was pretty freaked as I had read some studies which state that radiation causes problems to implants but it turned out o.k. My plastic surgeon, who has done many reconstructions, says that none of his patients who have needed radiation to their implants have had problems. Interestingly, many of the later studies I read had found no problem with post-implant radiation. Maybe there is some difference between the implant materials or the type of radiation between years ago and today? It doesn't seem to make sense - but then again the outcome for me was fine so I'm not complaining!
-
Thanks Kerry1000! I'm so glad your radiation turned out well. I had #12 today and so far so good. My rad onc says I'm going to be "toasty" - sounds so yummy!!
My plastic surgeon told me not to worry, she'll fix it.

I had a bilat mast / immediate reconstruction with implants and Alloderm. I didn't expect to have rads but did...micro mets to 3 nodes and angiolymphatic invasion. I had my 19th treatment today. So far, so good. I don't expect to have problems with the implant, it's the skin and tissue surrounding it that I'm worried about. Everything seems to be holding up well for now ... 9 more to go.

Hey Twink!
My reconstruction was with Alloderm as well. Sounds like I'm right behind you, I had #14 the day you had #19. I am receiving the bolus everyday; do they use this on you? My rad onc wants to do 25 treatments and then reassess to see how my skin is holding up. She wants to do more whole breast rads instead of boosts since I have had two cancers in the same area. So far, so good - my skin is looking a bit sunburned and itches but not too bad - YET!!

oops...missed this one Dawn. Sorry.
I was getting the bolus every other day during the regular treatments. Now that I've moved on to boosts (#2 of 5 today), I get the bolus every day. Skin continues to hold up pretty well.... hoping the reconstruction holds up too. Almost done, right?
-
No problem Twink - me too!! LOL
Can't always remember where I've added comments and I get tons of emails already so I don't do the favorites click.
You are almost done for sure! How cool is that?!?!?!
I had the reassessment w/the rad onc - She is doing things different for my boosts - they did a ct scan today and mapping will be on Monday - had #26 today of the supraclavical, breast and lymph node area - last supraclavical. Tomorrow (making up for Tuesday) will be #27 with just the breast/lymph node area, Monday #28 will be the same. Tuesday I'll start the "boosts" but it's to most of my breast since I have so much scar tissue - I believe it's the whole breast rads as opposed to whatever they do for most boosts. whatever. I am so hoping it'll be a total of 33 and done which will be the 14th!
I think the reconstruction is fine........................hopefully both of ours will be in the future as well.
Have a great weekend!!
Dawn
