5 day (“Fast Forward”) radiation schedule
I am discussing radiation therapy options with my rad. oncologist. He is recommending hypofractionated breast radiotherapy lasting one week. It’s obviously more convenient. However it doesn’t have a super long track record. It had been studied in the UK under the name “Fast Forward”, a follow on to a study called FAST
Is anyone familiar with this? Had it? Thoughts?
Is 15-16 treatment most typical?

Hi Kiki12345678 - sorry you had to join us here but we are a very supportive group. I had five (5) sessions of APBI (accelerated partial breast irradiation) in January, 2021. There are a couple of articles on this site that you may find interesting

I too am discussing accelerated partial breast irradiation with my radiology oncologist. I am scheduled for a SLNB on December 2 and if they are clear, then I will be a candidate for partial breast radiation. I was also told that I could have whole breast in a 5 day course if necessary. He sited the UK FAST study and the latest guidelines during my consultation last week. Convenience is a big factor with me...getting it over with in one week. I would like to know if anyone whose had this experience personally and has any tips or feedback to share?
My diagnosis was originally DCIS in May, but after my lumpectomy in August, the pathology was ambiguous, and there was a 2nd, then 3rd opinion which showed an invasive component. So now SLNB before radiation. Hoping to get this all complete before the end of the year.

I had 5 days radiation regimen in march of 2020. I had DCIS and surgery and after that had this option. I took it.
I am happy how it went. The only thing I was warned that I will be receiving not full breast radiation but part of the breast, where my DCIS was found and around that spot. It was explained to me that I am getting the same amount of radiation as if I was getting 3-4 weeks, but it is not enough time to radiate the whole breast.

I started a five-day course of whole breast radiation yesterday and will finish on Friday. I met with two radiation oncologists, one of whom recommended the five-day approach and one of whom recommended the traditional four-week approach (15 days of whole breast radiation and five days of a boost).
One of the reasons I hesitated to go with the shorter course is that outcomes data is only available for five years. (I'm young--and optimistic!--enough that I'd rather talk in terms of 40.) The four week approach has a longer track record. Ultimately, though, convenience won out.
I would be happy to keep you posted on how things unfold for me of that would be helpful to you. I have found decisions about radiation--whether to do it at all, five treatments versus 20, whole breast versus partial breast--to be the most difficult yet. At the same time, I recognize that I am lucky to have choices. I wish you all the best in making your own decisions.

Thanks very much for replying. I'm otw to appt this morning with radiologist, facing apparently the same options as you had- though I'm not young-but in excellent health for my age.
I'm curious-did you see 2 radiologists in same practice or get 2 opinions from different sources? I'm at a major cancer center, where people flock to from around the world. I'm trying to figure out today if there's an institutional pov on this question, or does each doc recommend what he/she likes best. 🤷🏻♀️

I consulted with radiation oncologists at two different major cancer centers, both of which are destinations of the sort you describe. I do think institutional culture factors in, with some places more inclined toward the tried, true, and proven and others more open to the new and emerging.I’m in the waiting room now, but I’m going to send you a private message with more detail after my appointments. How was your experience with the 5-day treatment? Was it full breast or partial? Now that some time has passed, are you happy with the cosmetic results? I am scheduled to have accelerated radiation starting December 13 and wondered if you can share any wisdom with me. I have a simulation session this Friday Dec 3rd.
