Worried About My Diagnostic US Report

Hi,

I'm 46 and received a callback for asymmetry in left breast, BI-RADS 0. This was with a 3D Mammogram and compared to my previous four 3D mammograms. I am getting married in 8 days and was expecting/hoping to go get the diagnostic done and move on with my life and this final week of pre-wedding anxiety. :)

I received my report this morning with a BI-RADS of 4B. Yesterday, the radiologist told me it was BI-RADS 3. So, that is a change. I was wondering if anyone can take a look at this report and let me know how worried I should be. Thanks in advance!


Impression

Several well-circumscribed anechoic and hypoechoic lesions of the left breast are indeterminate. Options of six-month follow-up and biopsy were discussed with the patient.

BI-RADS CATEGORY: 4B - Suspicious.

RECOMMENDED FOLLOW-UP: Options of biopsy and six-month follow-up were discussed with the patient. The patient was interested in biopsy. The patient indicated that the biopsy would be performed in Boise, ID. I expressed to the patient that 1 to 3 lesions
could be sampled, depending upon the preference of the radiologist performing the biopsy.

Narrative

BI BREAST ULTRASOUND LEFT, BI DIAGNOSTIC LEFT TOMO

HISTORY: Abnormal breast finding

COMPARISON: August 5, 2020.

TECHNIQUE: Left-sided Spot compression cc view. Left CC, exaggerated left CC and ML views obtained. Tomo utilized.

FINDINGS:

MAMMOGRAPHY: There are persistent partially circumscribed asymmetries within the outer left breast, only noted on the cc view.

ULTRASOUND: Within the left breast at the 12:30 position, 4 cm from the nipple there is a hypoechoic well-circumscribed lesion measuring 6 mm. This could represent a fibroadenoma or a complicated cyst.

Within the left breast at 1:00 position, 5 cm from the nipple there is a simple appearing cyst measuring 2.2 x 0.7 x 1.7 cm.

Within the left breast at 3:00 position, 7 cm from the nipple there is a 9 x 5 x 9 mm structure compatible with complex versus complicated cyst.

Left breast 2:00 is, for centimeter from the nipple there is a wider than tall simple appearing cyst.

Left breast 2:00 position, 4 cm from the nipple there is a hypoechoic lesion with increased through transmission with small internal hyperechoic focus. This may represent an intramammary lymph node. Complicated and complex cysts are other differential
considerations.

Normal left axilla.

    Hi. The radiologist told you a birads 3 meaning you have an option of waiting 6 months to see if there is any change, or getting them biopsied. He had to put the 4 on the report so that your insurance will cover you. It is not that concerning that you would have to do it within the 8 days of your wedding. What a great distraction to have. Congratulations! I would try and enjoy yourself and either schedule the biopsy's after, or you do have the option of waiting 6 months to check again. Only you can decide which. Best wishes to you.

    We did talk the option of biopsy or wait 6 months. I would definitely not be proceeding with anything until after the wedding/honeymoon.

    That makes sense about the insurance. I guess I was just shocked to see 4b.

    Hi Stash, just a note to say I know what it's like to be blindsided with a scary medical situation right when you're supposed to be planning a wedding, and I feel for you. Being able to wait 6 months is fantastic! If they were really worried about it they wouldn't allow that. Set it aside and enjoy this special time! And congratulations!
