Slowly being pushed out of job

I have been in a slow process of being pushed out of my job since returning after chemo and surgery two years ago. Before cancer, I worked easily 50 hours a week or more. I was constantly juggling many things at once, wearing multiple hats, etc. I no longer have the energy or the focus to work like I did before. I've asked for help, but never get any real solutions, and instead my managers and co-workers discuss hiring someone to do what I do, without actually saying they are replacing me.

I'm not sure where to turn to for help. I don't have the energy and sustained mental focus to start a new job in a similar position elsewhere. I talked to a couple of career counselors a while back, but only the initial consultations, and my take away was that they were more experienced with people who had money and connections who were bored with their lives and wanted someone to help them make subtle changes rather than people with serious legitimate issues. At the same time I have found other legal and career counseling resources, but those are for people with much more serious problems than I have.
