BIRADS 4: Suspicious Lymph Node

Hello everyone!

I am new to this community and find myself trying to gain some information on my recent Ultrasound results. But let me catch you all up.

In Feb. 2021 I noticed some swelling under my left armpit that has lasted about 2 weeks before seeing my GP (Mind you it was unable to be felt). I get an ultrasound which indicates 2 swollen lymph nodes - possibly reactive but to see a general surgeon if clinically applicable. I see the surgeon in March who then stated that since the lymph nodes aren't able to be felt, it's probably just reactive and the swelling under my left armpit is possibly due to breast tissue. I am then scheduled a follow up ultrasound in July 2021.

Within that time frame, I have had off an on fatigue and itchiness here and there. I was also sent to an Infectious Disease doctor due to some elevated CBC results. She ran a panel of tests all ruling out anything that could cause my lymph nodes to swell - I also wasn't sick with anything during this time as well. I had CT scheduled with her for my chest and now neck because my lymph nodes also became swollen in my neck but have since gone down. The CT has shown no axilla lymphadenopathy but a few sub centimeter nodes in my neck (likely reactive) back in May 2021.

I did my ultrasound for my follow up of my left axilla which now states:

Left BREAST BIRADS CATEGORY 4: SUSPICIOUS

RECOMMENDATION: If there is no clinical reason for reactive
lymphadenopathy, this would be amenable to ultrasound-guided biopsy if
clinically appropriate.

IMPRESSION:
1. Persistent mildly enlarged left axillary lymph node with eccentrically
thickened cortex measuring 1.1 x 0.9 x 1 cm.
2. Additional benign-appearing lymph node within the left axilla has
improved in size and morphology.

COMPARISON: Left axillary ultrasound dated 3/11/2021

FINDINGS: Targeted sonographic evaluation of the area of interest within
the left axilla was performed. There is a persistent mildly enlarged left
axillary lymph node with eccentrically thickened cortex measuring 1.2 x
0.9 x 1 cm (previously 1.1 x 1.2 x 0.6 cm). The cortex measures 6 mm.

A second benign-appearing lymph node within the left axilla measures 1.5 x
0.9 x 1.2 cm, has improved in size and morphology. No new suspicious lymphadenopathy.

Based on these findings, how would you interpret these results?

    I don't know about interpretation Legend though the persistence is bothersome. I probably would want to move forward with biopsy if I was in your shoes - just to know what's going on.... Unfortunately that's about all I can offer.... Please let us know if you follow up on this - it might help others too....

    Thank you for getting back to me! I went ahead and opted for the biopsy. However, the general surgeon who requested this follow-up didn't feel the biopsy was necessary due to me being "young" and the size to not have changed. When speaking with a radiologist who will be performing my needle core biopsy, she was a bit hostile towards the situation because it was stated that size doesn't always necessarily matter. In my case, the lymph node I had back in Feb 2021 now has changes within itself (eccentrically thickened cortex) which warrants a biopsy. I have been getting the run around with these doctors for quite some time now but have learned about advocating for myself.

    Just a little update! Had my biopsy done a few days ago and now for the worst part... waiting.

    I'll check back in when I know more.

    Update!

    To anyone that is reading this:

    I am still waiting for my biopsy results and got word from a nurse that they had sent my biopsy out to a different lab for a consult. I'm not too sure if that is a good or bad thing, but here I am still waiting for these results to come back which was said it could take up to a couple of weeks.

    Waiting is so hard!!!!!! Be extra good to yourself, distract when that feels good but don’t judge yourself if you do get caught in a racing thoughts and/or panic. Hoping for benign results!

    Geez on the waiting Legend! That would probably worry me if they were sending out to a different lab.... Hmmmmm... hopefully that just means due diligence given their initial lack of concern and then the change noted.... Thanks for updating us. Obviously we'll be interested in hearing once you get results. Ditto on what Kate said too...

    Hi @Legend808, just wondering what the outcome of your biopsy was. I'm in almost the exact same situation with lymph nodes that a[pear reactive but have been unchanged for 8 months and have my biopsy scheduled for Tuesday.

    Jlt814, this thread hasn't been active since early August, and neither has the poster. That usually means it was benign and she didn't bother to come back to let anyone know. You might want to start a new thread with any questions you have, or just to introduce yourself to the community here.
