Intraductal lesions - need advice in South Africa

Hi all. Im 39 and live in South Africa and would love some of your informed opinions on my issue. I had a fibroadenoma confirmed with biopsy last year. At follow up in april this year, doctor did ultrasound and found two spots which he suspected to be intraductal lesions. He thought it could possibly be intraductal papiloma or DSCIS. i had a single incident of spontaneous bleeding from right nipple in april and nothing after. I have since been referred to the state hospital and i saw a doctor couple days ago. She did physical exam and said she did not feel the lesions. Are they always palpable? Can this be just debris within the duct? I am scheduled to do another ultrasound at month end and "if" anything suspicious is seen biopsy will be done. On original ultrasound it stated:

Hypoechoic nodule at 5o clock, 3cm from nipple measuring 4 x4 x2 mm in cross section.

What are the chances that this is just debris, papiloma or DCIS? will it still be there in next ultrasound? Thank u all.

    Hello,

    I just wanted to send my love and light your way. I also am in this weird period of waiting and also 39.

    Did they give your a BIRADS score? When I looked at the elements in a BIRADS score it basically tells you what the radiologist best guess is. It sounds like on ultrasound they don't think it's highly suspicious or they would proceed to biopsy but it is something they want to watch. So maybe a BIRADS 3? (I'm def not an expert- very new to looking into things as I wait for my biopsy). Although being in South Africa I'm not sure if it's similar, same, or how they distribute health care resources which may make a difference.

    Lesions are not always palpable. I read until they are 1-2cm they can be missed. My 1.5cm nodule was notidentified as palpable by anyone of the many people palpating my breast (3 people?) However, they are fairly dense and moderately big (34DDD/DD ish) on a slimmer frame (140lbs/5'6).

    hi. Thank u so much for your response. We actually do have this system here. Original ultrasound was birad 3 with 6 month follow up. But im referred to state hospital who will now do a repeat ultrasound. However, im wondering if something that shows up as lesion on ultrasound can be debris?

    Radiologists can suspect what lesions are based on characteristics they see, but you really can't get that definite answer without a biopsy. A papilloma can cause bleeding from the nipple, so that could be a possibility. And as stated above, lesions are definitely not always palpable.

    I don't understand why you weren't recommended a biopsy after the first doctor thought DCIS/papilloma was a possibility. Going to another facility to have a doctor feel for it doesn't seem all that valuable. I have to think the lesions will still be there and they will recommend the biopsy or biopsies at that time. Good luck and let us know how your next ultrasound goes!

    EDIT--Oh, ok...you had a wait and see followup. Makes sense now. :)

    the reason i could not follow through with the original doctor was because of financial reasons. I had to be referred to state facility. Two ultrasounds were originally done privately. The first one missed a second lesion picked up by the second one. Now they will redo the ultrasound and see what they find. So you can understand why i am anxious that a third ultrasound could also be inconsistent. Thanks for your responses. I also feel the lesions will still show on ultrasound and hope they biopsy.
