Immediate recon with implants issues
Hi I had double nsm with immediate reconstruction one week ago. Now having complications on one side....blisters and necrosis. Anyone else? Any tips? So scared. Thank you for thoughts

I had bi-lat NSM with expanders placed rather than direct to implants and while it was a bit of a saga things did work out. I would suggest trying to find a hyperbaric chamber for wound healing as that can be helpful for some with this issue, but in the meantime try supplementing with zinc and bromelaine if your doc approves, and aim for 100g of protein daily to assist in healing. I used Silvadene cream by prescription to help the superficial skin and a petroleum gauze dressing over the skin. Depending on where the necrotic area is could you also use a wound vac?

I would call your doctor to see what they can do for you. I too had some necrosis with my nipple-sparing, direct to implant bilateral mastectomy over a year ago. My doctor prescribed nitroglycerin ointment for the necrotic nipple areas, and I'm happy to say that ultimately everything healed up nicely in my case. I did not have any necrosis at the area of incision, though, so I'm not sure how they might handle that issue.

thank you so much. I have near incision so that will necessitate another “small” operation to remove and restitch....bigger concern is area on breast itself. If it decides to heal I’ll just need the “small” procedure but if it doesn’t by next week then we will need to remove the skin, likely the implant and do a graft. So scary so trying anything to help it heal! Thank you again!

thanks so much ! I’m a week post op so maybe that’s why they didn’t mention the hyperbolic? Am def taking zinc and eating high protein!!! Thank you so much!

I will say that total resolution is a slow process so don't be discouraged if it seems like it is taking a while for the necrotic areas to resolve. Not all docs are aware of or endorse use of a hyperbaric chamber, but could also be wanting to wait a bit. Full disclosure - I did not personally use hyperbaric therapy, but other members on this site have and feel it was helpful. You can put hyperbaric chamber in the search function to read their posts.
