How to add an explanation tag to my diagnosis

I cannot figure out how to write an explanation tag near my diagnosis like I see so many other ladies have done. I am hoping someone can let me know where that option is. Thanks!

  Thanks for asking that question - I couldn't figure this out some time ago so "had to" leave some stuff off. Didn't think to ask. With this new info I went and added the info! : )
