Nipple/Areola Weirdness

I am quite upset to be even posting here. But I feel comforted to post where there are so many beautiful, brave women. I admire you all for your incrediblestrengths as well as weaknesses you fought through to emerge stronger.
I'm 55. I've never been a mother. Never been on hormone replacement. Newly meno - 16 months. I'm in good shape, don't drink or smoke, but have been through a decade of extreme stress. My mother, aunt and a cousin had breast cancer. My cousin- it was inflammatory breast cancer and she past away from it. I'm going for genetic testing in coming weeks. Not sure when - Covid is making everything so difficult.
7 weeks ago I woke up with strange areola and nipple pain in my right breast. Looked in the mirror and was shocked to see entire irritated red nipple, swollen areola and 2 red streaks reaching up an inch beyond areola into skin. This literally happened overnight. Over two days the streaks were receding and the pain had stopped. A tele-doctor (due to Covid) prescribed Azithromycin and sent me for an ultrasound. Antibiotics helped a bit but a bit isnt good enough. I called a local doctor (I don't have a primary) and he said it's mastitis and put me in clindamycin and a topical antibiotic. It worked better - and I'd say the issue improved to about 75%. Ultrasound results came back as bilateral birads 3 - I have cysts in both breasts - probably benign- recheck in 6 months
Since finishing antibiotics, improvement has been at a crawl. The weirdness is that the nipple on my affected breast is just slightly warmer than the other by up to 1 degree during the day. Because of that temp difference, 50% of the time it looks weird compared to the other. Because of the temp difference, the affected one doesn't react the same as the other. It won't be as erect when cool. The nipple and areola is slightly darker than the other. It just doesn't look right. It feels normal most of the time but sometimes the nipple is very sensitive. At night after warm in bed, if I get up to bathroom and look, they are pale and relaxed and look identical. In morning, they look identical and temp is the same. When cold and erect, they look identical except affected one is slightly redder than other.
(UPDATE: I forgot to mention, in earlier weeks lymph nodes from the side of my breast to all the way up into my neck and even face were painful.)
I've had no eczema type rash. No discharge. No itch. Very little pain, weeks ago. No breast size change. No orange peel skin changes. No inverted or flattening nipple (affected one is smaller than other sometimes). No lumps under nipple. Only initial irritation and swelling that receded, remaining redness, intermittent sensitive nipple, and intermittent temp difference.
To me, I don't have any of the classic symptoms of Pagets or Inflammatory. But I'm panicking. The weirdness is really terrifying me. Calling doctor tomorrow and depending on protocol during lockdown, he may see me.
Has anyone out there had this kind ofweirdness??

I"m glad your ultrasound came back benign and you'll have the peace of mind they will follow you more closely with the 6 month follow up. Sounds like the other two docs were treating your for an infection - the warmth, redness and streaks alone are tell tale. It is also possible that has improved from what you described while not being gone yet. You may need more antibiotic treatment? Soooo glad you're putting in another call to a doc today.
With your family history and current issues and the fact you're getting older it is important you get a primary care. Not sure what the obstacles have been or are for you not to have one up to this point. Up until a few years ago I did not have a PCP for 7 years due to poor experiences and having multiple specialists who prescribed all my meds, lab work, testing etc. The less pertinent and "in between" appointments medical issues started popping up - I recognized the need for a primary thanks to an Urgent Care P.A. You're 55 so I believe that is important for you as well. Unfortunately we start having more issues as we get older - even for those who are generally healthy... Please let us know either once you've talked with another doc's office or been seen....

Hi Tenkma, and sorry you’ve been dealing with this.
To me, it does sound like you have had an infection/mastitis that is slowly resolving with the antibiotics. Breast infections (and non-lactational mastitis is seen here on the threads pretty frequently) seem to be notoriously difficult to clear up, often requiring multiple rounds of medication. I hope your doctor can see you in person. As to lingering color changes, etc. if you have had another kind of infection that involved the skin, they often go through a series of color changes from red, to less red, to dark and so on before completely clearing. Lingering warmth and sensitivity might indicate there is still some degree of lingering infection.

I've found a primary physician. I enroll with him tomorrow morning and will immediately request a referral for a biopsy. I don't want to sit on this hoping another possible round of antibiotics will work when I have a gut feeling it won’t.
It really had improved to about 75% to 80, but just yesterday I woke up and was crushed to find it very VERY red and a bit swollen again. All the improvement it had made, gone. I'm absolutely terrified.
There is still none of the eczema type symptoms or scaling or discharge or bleeding or anything like that, just redness, slight swelling and some sensitivity. But I've read stories of women who have only had the redness. I also read of nothing showing up on ultrasound, so my results that were a relief now no longer seem to matter at all.
Can't eat or sleep. I worry about who will look after my little dog if I have to go have surgery - I have no one and she is everything to me. I worry if I can even get the treatment I may need with the pandemic and outbreaks at the local hospitals. I look at the odds and how rare Pagets is and I wonder how this could happen?

Ren- QUIT reading the stories. I'm glad you're going to see a PCP. Still no reason to panic. Really - stay off Google.

"Gut feelings" and "I read" are the worst team of medical practitioners in the world, and should not be listened to at this point, especially since you've already had antibiotic treatment that mostly worked (which would not be the case with cancer) and an ultrasound.
Good luck with your appointment. I'm not sure if you can get a biopsy-on-demand since nothing showed up on the ultrasound.
