Hi just had Mri results after having mammo and ultrasounds for every 6 months for last 20 months . Been having breast swelling and bloody nipple drainage spotting . So last week was new 6 months test ultra sound came back with dilated ducts and complex cyst at 2 o’clock 6 mm unchanged . So mri due to blood. Mri results comes back BI-RADS 4 now and said 3 enhanced lesions largest 6 mm smallest 4.5 mm . And mild background parenchymal thickening in both breast . See General surgeon for follow up .

So my question was I got more than 1 lesion ( not identifies ) Why lesion ? I am lost what does that mean lesion . And enhanced?

I have contacted cancer center to seek a surgeon there . I don’t want a general Surgon.

Does this sound possible cancer ?

    The following is a break down of a Birads 4 score:

    Category 4: A score of 4 indicates suspicious findings or abnormalities. This level is where concern for breast cancer risk begins to increase. In this instance, there is a 20% to 35% chance of cancer. To verify, your doctor will need to perform a biopsy to test a small tissue sample. This score is split within three additional categories based on the doctor's level of suspicion:

    • 4A: Low suspicion for cancer or malignant findings (more than 2% but no more than 10%).
    • 4B: Moderate suspicion for cancer or malignant findings (more than 10% but no more than 50%).
    • 4C: High suspicion for cancerous or malignant findings (more than 50% but less than 95%).
    Contacting a surgeon at a cancer center is the correct path to start on.

    A "lesion" is catch-all term that simply means that something is seen. The definition of lesion is "any damage or abnormal change in the tissue." It could be benign or it could be malignant - the word 'lesion' doesn't indicate either way.

    It's unusual for breast cancer to develop in 3 different spots at the same time, so the fact that you have 3 lesions might be in your favor. But from the description, it's impossible to know whether one or more than one are cancer or not. That's why the imaging has been assigned a BIRADs 4, which means that a biopsy is recommended.

    thank you that helps a lot . I didn’t know and google offered no help on the matter . Do you know if they do a biopsy if they will biopsy all 3 lesions ?

    Goin to make a appt with a breast cancer center to have them review all scans . I was surprise that the mri didn’t show the cyst which was seen in ultra sound and classified as a multicluster complex cyst . So I don’t really know if there is the cyst along with 3 more lesions . Not happy that the reports are not clearer .

    hi thank you for this information . The radiologist only places it at BI-RADS 4 no other info .

    Last month on ultrasound of breast they classified as BI-RADS 3 with no other info . So I went from 3 to 4 in 30 days time .

    Still disturbing . My mother just passed from breast cancer and so did my g mom . And I have already have had thyroid cancer in 2013. Which was not fun. So it scared me to think of things ya know .

    I am hoping the breast cancer center is more proactive in giving a definitive answer than the imiging center I had gone too for my other test.

    Is there any questions I should ask the doctor when I see them ?

    Did you have an ultrasound 30 days ago and then again now? Or are you comparing the results from the ultrasound 30 days ago to the MRI you just had? Each modality of imaging sees into the breast differently and shows different things. So when something shows up, it's not uncommon to have different BIRADs scores on each modality, even if the screening is done on the same day. I've had that happen more than once. I've had BIRADs 4 on a mammogram and BIRADs 2 on an ultrasound done on the same day. One time I had a BIRADs 1 (which means nothing at all is seen) on a mammogram and a BIRADs 4 on an ultrasound done only one week later.

    I don't know if the surgeon will want to biopsy all three lesions or just one. It may depend on how similar they look to each other (would the results on one be indicative of the others?) or if one looks much more suspicious than the others (test the most suspicious one).

    With your family history, have you had genetic testing done? Since you've had thyroid cancer, you should have full panel testing, not just BRCA. There are other genes, such at the P-TEN gene, that increase risk for both of those cancers. It doesn't mean you have that genetic mutation - on my maternal side there are several cases of breast cancer (my mother and myself included) and several cases of thyroid cancer (my brother being one of those cases) and yet I've tested negative twice now for the P-TEN mutation. But it's certainly something that you should check out, if you have not already done so.

    the test were done at separate times they put me on the every 6 month cycle . This last one is when the bi rads went up .

    I have not had those test but will ask the doctor on next visit at cancer center .

    Update 10/1/20

    Hi everyone , Hope this is finding everyone in good Spirits today. Wanted to update and let everyone know i Had my Biopsy's done yesterday. The Doctor took Core Samples from 2 Solid Masses in the "Retroareola" area they were small masses but the doctor said they looked suspicious and meet the requirements to biopsy. The cyst she said she was not biopsy because she felt 100 % confident it was just a cyst. The doctor did a great job making sure i had no pain thru the procedure . I was told that the Biopsy results should be in 2-4 business days . But she is hoping Friday, but should be no later than Monday .

    I am nervous and scarred i am sure everyone goes thru this . I think my brain is numb in the realisation that it is 50/50. But i feel as if my heart knows what the out come will be. and strangely enough i am not panicing.


    Has any one been diagnosed with Retroareola Cancer ? is this area more hormone based Hers2? I am trying to read up on the different hormones related to this area .

    Can any one share with me there out comes with Cancer in this region ? i understand it is with in 2cm of the nipple area . I know one of the masses was 1cm from nipple at 1 oclock, and the other was further away so not sure if that one will be classified as retroareola region .

    I will try to research on this forum to see if there is any info on what kind of cancers like to brew in that area .
