Flap or Implants?

Hello fellow BC survivors!

Looking for some insights/opinions....I had a lumpectomy, re-excision including SNB (clear) & whole breast radiation in late 2018 (for DCIS), and was just diagnosed with a recurrence of DCIS in the same breast. I'm leaning towards a bilateral mastectomy and immediate reconstruction, but am hearing so many pros/cons about flap options vs implants. It seems like so many women have had issues with implants, resulting in pain, illness and follow up surgeries.

I'm 55 yrs old, 5'5" and 155lbs. My BC surgeon recommended a PS who specializes in DIEP flap; he and his practice partner are both microsurgery specialists who operate together to reduce surgery time. When I saw him, his first recommendation was T/E & implants (I think he assumed I wanted to stay same size, about a D cup) and he thought I didn't have enough tummy fat. I told him I was fine with smaller breasts; he said I may be able to a DIEP and then supplement with fat liposuction later down the road. And, if I really didn't like the results, he could always do implants later. I've got a metal rod in my spine from when I had scoliosis as a kid, I'd prefer not to have any other foreign objects in my body:-)

    I had DIEP flap reconstruction at the same time as my bilateral mastectomy. I had a couple of hiccups, including a collapsed lung, but overall my recovery went really well.

    I'm 5'4" and weighed about 150 at the time of my surgery. The microsurgeon who did the DIEP flap called me obese - which I wasn't. (Great surgeon, terrible human being). I did, however, carry all of my weight around my midsection. They were able to construct two D/DD breasts. I wanted C's, but that's another story.

    It's a very big surgery. Mine lasted 12 hours, even with two of the best microsurgeons in California working in tandem. The recovery period is somewhat longer than for implants. That's the downside.

    The upside is that I woke up from my mastectomy with two warm, natural looking and feeling breasts. I never felt "butchered " the way many other women have. Once I get my nipple tattoos, no one will ever be able to tell my breasts are not natural natural. They are that good.

    I wish you the best. I hope whichever surgery you choose gives you exactly the results you are looking for .

    Hi LizLynne. Last year, I had a nipple skin sparring bilateral mastectomy with TE’s immediately expanded. I was a D cup at the time & PS said I had a Iot skin. I absolutely hated the TE’s and radiation only worsened that. I am currently recuperating from a Diep & Lap flap. I’m 5’2 & weigh 150. It was a 12 hour surgery. I spent 5 days in SICU and 1 day on a regular floor. My PS really doesn’t like doing implants when radiation has occurred. Of course he would,if implants are what the patient wants or where flaps are not doable. I’m currently a DD but PS said I will lose about 25% when all said & done. Healing is going well & looking forward to getting back to normal activity. Whatever you choose, it’s a very personal decision. Best of luck to you on your journey.

    Thanks Allisat, glad to hear you are healing now, hopefully you're back to normal activity soon! I think I'm going to get a 2nd PS opinion if I have enough tissue for flaps on both sides and then make a decision. Take care & best wishes to you too!

    I am 27 months post DIEP. I had double mastectomy with tissue expanders in Nov 2018 and DIEP flap surgery in April 2019 (post radiation). I had revisions in Sept 2019 and Jun 2020. The initial flap surgery was 9 hours. The revisions were both short, outpatient procedures.

    I was initially thinking of having implants but my PS told me size would be limited with implants because my ribcage is narrow even though I had large breasts (36DDD). DIEP flap surgery is a big surgery witth a long recuperation period. However, when all is said and done you are left witth your own natural flesh. I was able to stay the same size. I had some issues with healing after my surgery but everything did heal and I did not have any infections.

    I was 52 at the time of the surgery. 5'3", 195 lb (roughly).

    @trishyla Your results sound fantastic! Would you mind PMing me the names of the surgeons? Was it covered by your insurance? I'm in CA too. Seems a lot of PS here don't even take insurance.

    I was planning on DIEP but very ironically I ended up not having enough fat in the right place so we started exploring SGAP. I needed an MRI though and so had to get rid of the expanders and I just did not feel I was physically strong enough to handle bilateral SGAP at that point in time as I was still weak from chemotherapy and the BMX so I had the expanders switched out for silicone implants. The exchange surgery wiped me out and set back the energy gains I had made in the preceding months and that is a significantly less invasive surgery than SGAP, so I feel I made the right decision at the time.

    Maybe I will still have the SGAP in the future though. Implants have a shelf life.

    Thanks for sharing your experiences!! For what it's worth, I ended up going down the TE/implant path as two PS docs said I didn't have enough tummy tissue. On the positive side, I was released from the hospital same day. Due to COVID, I was not permitted to have anyone come with me so I was really glad I did not have to stay overnight. DIEP would have been 3 or 4 days....

    I had TEs put in during the BMX and it's been about 2 weeks. Had my first fill last week. The fills don't hurt but are odd...like going through puberty in 20 minutes. So far so good.

    Liz - I've been on the boards for a LONG time and that's the best statement I've ever heard about filling the TEs. Never thought about it before, but you're absolutely right. Glad you're doing OK.

    ...like going through puberty in 20 minutes.

    Just PMed you Trish--- by the way, did insurance cover everything for you? Here's where I'm at-- one of my silicone implants might have "ruptured", suspected something happening, but found out at my mammo/ ultrasound/ BS appt last week. So now I have to have a MRI in 2 weeks. But I don't have cancer! My implants are about 15 years old...Here we go again... Over to the reconstruction boards. I really want a diep but I'm 68 now. Had L mastectomy w/ implant and R breast lift with small implant in 2005. First reconstruction in 2004 had to be re-done, very uneven. The double mast ladies that get reconstruction have it better in terms of symmetry. My re-do was pretty symmetrical until past 5 years or so when good breast has started sagging. At the time I did implants I was happy with the easier recovery but I guess since it had to be re-done maybe I should have just done the flap in the first place.
