Fat grafting for implants and liposuction
So 12 years when I had my DIEP stage 2 surgery , my PS said that I could get liposuction at the same time if I wanted for a bit more money. Then I was younger and had more stuff to do and wanted to minimize downtime so I said no.
Now I got the second surgery with the implant reconstruction. During implant exchange I would like to do fat grafting to make the breast softer. And once again there is a possibility to do liposuction. I can now afford more down time being retired so I am seriously considering this especially since I will be taking Arimidex which I would guess could cause me gain weight by lowering estrogen.
So just wanted to see if any of women here did liposuction along with the implant exchange and hat was their experience.

My PS does lipo 3 months after the exchange surgery.
I had my outer thighs done and I'm so glad for that!! I think it helped my boobs somewhat, but I think I need another lipo session to fill some folds in my implant.

I did FG with exchange surgery. I have to say the FG site (outer thighs) hurt way more than my breasts/incisions. I was very sore and bruised for about 2 weeks. I am very thin and was told they could not get much fat but did get enough to fill in an indent. I don't have any indents so I guess it worked but I still do have rippling. Wish you the best with your surgery!

I am 4 days out from exchange and lipo. Recovery is going great, just wondering about compression. I bought a couple of sports bras to wear rather than the post surgery bandaging. I still have on that huge wrap for the torso where fat was removed, only taking off for showers. I understand the bra keeping things where they should be while fat grafting settles. Why the compression for the torso? There really isn't that much swelling. Any thoughts? I really feel so done with that wrap!!!!!

My surgeon said I would need to wear them . And the nurse said to try to do the surgery after the summer since it is uncomfortable to wear the compression wraps. I found this explanation:

I had lipo on my stomach and legs and I absolutely needed it! My surgeon doesn't recommend breast compression garments, so I only had to wear stomach/leg spanx.

I only had to wear a Spanx type garment for 3 days and then whatever was comfy. I chose bike shorts for another couple of weeks and that was it.
Compression is needed at first to control bleeding and swelling. Excessive compression is not good for the recon, as the fat needs a chance to graft to the tissue.

I just had my fat grafting on Thursday. He took from my abdomen and sides (the love handles). Filled a dent on upper right and then along both inner and outer sides to try and tackle some ripples.
He didn't require me to wear a compression garment but I find it much more comfortable to do so have been wearing since first night. It keeps some pressure on to not only keep swelling down, but feels nicer on the belly incisions (I have two, just near my hipbones) and is more supportive overall as I move. I have a couple pairs that go from under breasts to mid thighs but are more mesh on thighs so cooler to wear even while sleeping. Wonderbra brand I think.
I can't feel any pain or soreness in my breasts at all though they are swollen and bruised - 4 small incision sites. I am just wearing my regular surgical bras that my surgeon put me in after each of my surgeries. They are stretchy with very mild compression I guess just to keep swelling down and add a bit of support.
I am quite thin so was worried about what he would get and how it would look but he did a good job it looks like and my mid section so far looks good. It's a little weird how much a difference taking a little fat out makes. I can see where he overfilled in breasts a little so I will see how it takes - apparently if you are thinner the fat may take better as it is more stubborn so we will see!

No, they did not flip me. I only have two incisions for the lipo part - one just above each of my hip bones - and so he maneuvered the cannula entirely from those two points. I can see the bruising where they just ran the cannula from the incisions back toward my side and back - like a wide V. And my bruising runs up a bit vertical along my side toward my lower eibs

Hi all! I am doing my prophylactic double mastectomy in a few weeks (BRCA+). My surgeon is recommending a nipple sparing mastectomy, TE and later exchange for implants (under the muscle). I am thinking about asking my surgeon about fat grafting as well. When would fat grafting happen (during initial mastectomy, during exchange, way after the exchange, or a combination)? Does fat grafting help with skin sensation after mastectomy or no? Also, I'm a bit concerned about the implant being under the muscle - I know some have chronic pain that they attribute to this. My surgeon said it needs to be under the muscle for better coverage of the implant. Is that still the case even with fat grafting? Could fat grafting provide enough additional coverage for the implant to sit above the muscle? Thanks so much for your guys input based on your experiences!

If you are curious about prepectoral implants, I would strongly suggest you seek the opinion of someone who does a lot of them. It sounds like your PS isn’t one of them, based on his answer, which is technically true, but leaves out the fact that prepectoral implants are generally covered with a dermal matrix of some sort, as well as grafted fat.
Many people can do the mastectomy and placement of implants in one step. That is easier with prepectoral as well.
Don’t be afraid to have all your questions answered and curiosities satisfied before proceeding. You want the best and most mind-easing solution for YOU.

Hi I only had 1st stage done and havent had exchange yet.
I also considered under and over muscle. I decided to listen to my PS and do it under muscle to protect implant. It kind of makes sense there is less possibility of rapture if the implant is protected. He did howevere said that he can do it over muscle if I wanted too. I think that for some pope who are very much into sports over muscle works out better.
Fat grafting as I understand it will not help with the sensitivty but will make breastws look more natural with the soft fat on the top.

My PS did not recommend post surgical bra & compression garment. He didn’t warn me about bruises. I founded I had them at home, there is no Incisions on my right side hip only hip & ribs where they are.

Had BLM 2017. Second breast cancer.
I had three fat gratings to my right breast. They took the fat from my stomach area.
Since this breast had cancer 14 years ago and I had chemo and radiation, the skin is thin and isn't keeping the shape that the doctor tried to achieve. Radiation is not kind to breast tissue.
Everything was covered by my insurance both times.
Had Implants replaced, opened up to clean out an infection and the three FG's.
Going back to have the right Implant replaced because of a small rupture.
They are silicone and before my first fight with BC, I had breast augmentation that were saline. I loved them. Had them for 19 years.
I was thinking about having both of the silicone removed and replaced with saline. Not sure how they would feel.
