Breast Rash, no answers Fear of IBC
I am looking for help, support, guidance, and anything you all may have to offer. A little over a week ago I undressed to shower and had a bright red rash on my right breast. It was not painful but appeared over the course of the day. Being a Friday night, I could not see the doctor until Monday afternoon. Over the weekend the rash went from bright red to a bruised looking inflammation. I still had no itching, swelling, or pain. The doctor that examined me was at a quick care style office as I was anxious and wanted to be seen ASAP. She looked at the rash, examined my breasts and said that since there was no lump, she really felt like it was just fungal and prescribed a cream. My right breast is larger than my left, but it has been much larger than my left since I breast fed years ago. I have large hair follicles on both breasts, and I don't believe that I have had any pronounced skin changes. My husband insists that I am worrying too much and that my breast is not any larger than it usually is, I am just noticing it more.
I had already consulted Dr. Google and was terrified that I was presenting with symptoms of Inflammatory Breast Cancer. I had read that this cancer is not usually associated with a lump, but more of a rash as I am dealing with. I used the cream twice daily until last Thursday when I followed up with my PCP. I had taken pictures daily of the rash and he felt like it was improving a lot but did not agree with a fungal diagnosis. He said I could discontinue the cream and thought it would clear up on its own. I had a few instances where the rash seemed to spread around other areas of my breast, but this was while I was using the cream and has also gone away. I agree that each day the rash is better. It is much smaller and almost gone. I am driving myself crazy with fear and worry. As the rash has continued to improve, I have developed a dull ache in my right arm (same side as breast that been healing). It isn't very painful, but it is there, and I am now wondering if this is somehow related to my mystery rash. I still have no itching or breast pain, but I have pain in my right arm in my bicep and radiating downward.
After my first visit a mammogram was ordered as I am due. I am only 33, but I have a history of childhood cancer treated with chemo and radiation and a recurrence of the same cancer. As part of my continued care I started mammograms at age 30. I do not currently have medical insurance and the local breast center has yet to even schedule my appointment since I am not covered. We are self-employed and during open enrollment we did not find coverage that fit our family. We have since tried to purchase private coverage, but I have been denied coverage. I am scared, terrified, and extremely anxious. I check my breast probably 50 times a day for any new changes. I am not even sure at this point how much my arm hurts or if I am making this all worse in my head. At times I think I am starting to have a dull ache in my breast as well. Where would you go from here? If my rash is getting better should I just try to relax and forget about it? Should I try to push for more testing? Has anyone dealt with trying to find care when you are uninsured? I appreciate any tips you may have about finding further care or where to go from here. I have googled and read every article I can find. I have read many stories about how IBC can present. I want to find peace in the fact that my rash is looking better everyday, but I do not understand why I had it or why I now have this pain in my arm.
I understand that Inflammatory Breast Cancer is rare and can present in many ways. I am seeking medical help from my doctor's office. I am looking for any advice in where I should go from here. The anxiety and fear is crippling me and I could use support and advice. I am afraid something is being missed and I won't be able to get care since I am uninsured. Thank you for any help.

Hi theswankyhen and welcome to Breastcancer.org,
We're sorry you're here and worrying, but we're all here for you. First, stop googling! If you're curiosity and worry must be satisfied by research, make sure you're reading reputable sources such as here at Breastcancer.org, American Cancer Society, and the National Institutes of Health.
Second, trust in your doctor and her treatment -- especially if you're seeing your symptoms get better. IBC worsens rapidly and so having your symptoms slowly get better makes it more likely that it's an infection, as your doctor has suspected. For more information, see the main Breastcancer.org site's page on Benign Breast Conditions, specifically the section on Benign breast changes due to inflammation, infection, pregnancy and more.
We hope this helps!

theswankyhen, I am sorry you are going through this. I had a red breast that sent me to my family doctor. She sent me to a surgeon who did a punch biopsy that was negative. I then developed a lump and had IDC.
I have had chemo with women who have no insurance, so I would push for testing so you can get this figured out. Talk to your hospital and see what they can offer for those without insurance and, if they are not willing to help you, talk to another hospital. I wish I knew more, but I do know more than one person who did not have insurance. You may have to show them your tax returns to qualify for charity assistance, but they cannot refuse you.
Prayers and hugs for you. Lisa

Thank you both so very much for your response. I was originally diagnosed with cancer when I was 7 years old and I relapsed at 9. I spent four years of my childhood fighting hard to be alive today. I often worry about the late effects of all of my chemo and radiation, but I try to focus on the now and be thankful for all I have. My original cancer was on my left kidney, which I lost. It also metastasized to my lung at at that time. I received wonderful care and this April I will celebrate 22 years of remission. In 2018 I had another scare when tumors were found on my spleen and pancreas. They did a distal pancreatectomy and splenectomy and both tumors were benign!!!! I will never forget when they moved me to the oncology ward in 2018. I had done it before, but being an adult and understanding the fight made it a lot harder. Hearing that all of the pathology was clean was so amazing. My body is covered in scars from so many surgeries, but I am so thankful to just be alive! I think my complicated history has made me crazy this time! I feel like it looks so much better, but in the back of my mind I think about all of the weird things that have happened to me and get caught up in the what if's. That is never a good place to be. I am a 33 year old mother and have two young children and an amazing husband. With all of the medical things I have been through I just want to watch them grow and be healthy. That is what we all want, I know. I appreciate your support so much. I am going to stay away from google and try to just breathe and pray and focus on today. We are working with another insurance company now trying to purchase some type of coverage, but if that does't work I will still seek care. If my symptoms persist I will ask my doctor to investigate further. Thank you both so much. Huge to all!

swankyhen - Breasts are never symmetrical - although most women don't notice until they panic. If your rash is getting better, I'd move forward and try to put it out of your mind. However since your doctor was "the internet", If it continues to be troublesome, your symptoms should be seen by a dermatologist. That's who deals with skin (& hair & nails). As you've discovered - Dr. Google is VERY good at sending everyone into a panic. You can do some more research here (a safe site) and maybe it will help, but I'd stay away from google for now.

"If my rash is getting better should I just try to relax and forget about it?" Yes, that seems reasonable.
IBC normally presents with a number of symptoms. You have just one of those symptoms, and this symptom is getting better on it's own. That suggests that this isn't IBC. You've also been seen by two doctors, both of whom believe that this is a skin condition. As MinusTwo advised, if you are nervous or if the rash doesn't completely go away on it's own, you might want to see a dermatologist. If the dermatologist isn't certain what the rash is, he/she can do a punch biopsy, which happens to also be the way that IBC is diagnosed.

Thank you again for your support. My rash/bruised looking area is still here. It has looked better at times, but is very much still around. It has been here for over two weeks now. It hasn't grown in size, but it won't go away. I have been taking pictures daily so this will hopefully help my doctors see the progression or any changes along the way. Up close it looks like a grouping of ruptured capillaries and is a purple/pink color. My husband now agrees that my right breast is larger than my left, more so than usual we both believe. We never got answers on private coverage last week, but began working with another company to purchase coverage that is not private and will allow me to join regardless of any preexisting conditions. We have decided that until the new coverage is active April 1st we will pay out of pocket for any testing and doctors appointments to get an answer about what is going on. I am very concerned and don't understand why it wouldn't subside by now if it were a bruise or something simple. The color is not progressing like a typical bruise and seems to be stuck where it is now. The swelling also has me on edge. I still do not have any itching or obvious pitting other than some large hair follicles around both breasts that I don't feel are very different than they have been before. My breast is also not painful. The area doesn't seem to be spreading, but it just won't go away. I was told by the nurse today (thankfully a friend so we spoke on Sunday) at my OBGYN office that they will squeeze me in sometime this week so he can examine my breasts and get me any referrals necessary. As far as getting into a dermatologist I will also start calling tomorrow morning from what I hear that can be a timely process. I am scared. Reading through the benign breast conditions I don't see anything that fits with my symptoms. I have read from many IBC patients that is usually comes on quickly and spreads, but I also see many that say they didn't present with classical symptoms and some had redness/rash that came and went. I am crying constantly and not eating much. I have worked myself into a really dark place of worry. Thank you for this place to speak. I am praying for appointments this week and answers. I hope you all have a good week to come.

Swanky - if you don't already have a dermatologist, they often have long wait times for appointmens. See if you can get your GYN to do a recommend if they feel that is a good plan
-
Hi Swanky,
I understand your fear, but spending your time reading and worrying will not help! Don't google too much, and don't try to self-diagnose. It sounds like you are on the right track - getting Drs. appointments lined up and insurance taken care of, so you are doing what you can. Try to not worry about the unknown and instead focus on taking care of yourself while you are waiting for your appointments/tests/etc!

I went in this morning from my diagnostic 3d and ended up spending about 3 1/2 hours at the mammography clinic. They went through my medical history carefully and talked about past surgeries, chemo, and radiation and my benign mass that was removed before. We talked about the swelling in my right breast as well as the bruised area/rash that has been here for 3 weeks. They said they do not see any skin thickening, peau d'orange or nipple changes. She did see the swelling and rash. I was concerned about peau d'orange, but she said that I seem to have a similar look in my other breast in the same area so it was not a concern to her. They said my right breast shows nothing of any concern and looked great on the mammogram. They did find two spots in my left breast and followed with a u/s. The radiologist did not like the first images so they sent me to a second u/s. They showed me the two spots, measured and took the imaging back to the radiologist. They came back and said that he believes they are enlarged lymph nodes. They are deep and near my chest wall. They did not want to do a biopsy and said I should follow up with another 3D mammogram in 6 months to re-check the nodes and see if they have returned to their normal size. They said the rest of the left breast looked normal on the u/s. I asked why these nodes are swollen and was told they don't know, it just can happen. I also asked what to do next about my right breast rash and enlargement. They said I need to keep pushing for answers, but now that this testing is done it can rule some things out.
My question there is that I have seen so many of you say that mammograms miss IBC. They didn't even do an u/s on my Right breast, which was my concern. Is this normal and what could have been ruled out on my right breast through the mammogram?
I head to my Gyn appointment in the morning and have a follow up with my PCP on Friday morning. I tried to ask what it could be or if they have seen anything like it but they said they did not want to speculate on what it is, but I need to continue to seek a diagnosis. I did specifically speak to them about my IBC concern and they brushed it off and said that that they did not believe it was IBC or Paget's, which I didn't even bring up.
I ended up leaving with more concern than I had before. Now in addition to the rash and swelling in my right breast I have swollen nodes in my left breast. I also tried several dermatologists and the closest appointment I was offered was near the end of April. I am still scared and confused. I have a lot of questions for my appointment tomorrow and I am hoping between my Gyn and PCP they can get a dermatologist to take a look at me sooner than the end of April.
Does anyone have any advice on what I should do, questions to ask or where to go from here? Why would nodes just swell inside the breast? Is it possible that the swollen nodes in the left breast have anything to do with what is going on in the right breast? I have never had any swollen nodes on previous mammograms, but they did not seem concerned.
I hope you are all doing well. Thank you again for the place to ask questions and find support.

You have upcoming doctor's appointments. Keep them, and try not to worry so much. Ask your doctor if he/she thinks a punch biopsy would be helpful since it seems to be a skin issue.
-
I am going to keep updating here in case someone in the future finds this thread and anything here may be similar to what they are going through. As always any advice is appreciated.
I ended up receiving another call yesterday afternoon late. They were able to get me into dermatology and had me come in at 7:30 this morning prior to my Gyn appointment. The dermatologist was lovely. She actually had everything setup to do a punch, but changed her mind after seeing it herself. She said that it could be caused from anything from folliculitis to contact dermatitis and she doesn't know what it is, but she does not believe that it IBC or Paget's. She said that the discolored area is small and that after almost three weeks she would expect other symptoms like more redness, distinct swelling, thickening of the skin, nipple changes, peau d'orange etc. She said I only have one symptom of many that might lead someone to consider IBC and with this combined with my mammogram, she is not concerned. She said that the mammogram would have caught some changes as well since this has been going on for a few weeks. I have read conflicting information to this online, but I am going to try to trust my provider. She also said that it would not be improving like it is. During her exam she found a hair follicle that was swollen and open. She believes that it might be something bacterial and wants me on antibiotics for a month. If anything changes I am to call the office and they will get me back in ASAP and if not she will see me back in 1 month to re-check.
Next up was my Gyn appointment. I had two very extensive breast exams by his student and the doctor and they both think that things look pretty good aside from the discoloration. They said that my right breast is larger, but they don't believe there is much swelling. As was suggested this part of my anatomy is probably more prominent to me now due to concern. I still feel like it is much more pronounced, but they were very thorough. They picked through every aspect of my medical history, previous tumors, biopsies, chemo and radiation. Unfortunately mammography had not sent over my imaging or the report, but I was able to convey what they found. They did not feel the spots seen in the left breast on the mammogram and U/S and also said they they felt like the explanation was poorly explained to me. He felt like they were probably normal lymph nodes that were just caught on the imaging, not anything of concern. He is going to get with the radiologist and confirm and look through the report. There were no swollen nodes found anywhere on my physical exam and he said he thinks this is going to clear up and is not any cause for immediate concern. He is also going to see me again in a month for another breast exam. He agreed with the radiologist plan to re scan the left breast in 6 months to re-check the spots and 5 and 6 o'clock. So, as far as my rash in the right breast I am praying these antibiotics clear it up completely and I can move forward.
If they do not I feel like my team will be ready to help me and answer any questions or address any changes. I wanted to say thank you for insisting that not having medical coverage should not stop me from getting answers. First, I think we all need coverage, but I know from personal experience how hard it can be to find good, affordable coverage, especially if you are self employed. We are moving forward with a new policy that should be effective April 1st. It is expensive and it isn't nearly as good as what we had in the past, but its insurance. Going about this self pay has not been terrible. My doctors office helped me use mdsave to get amazing rates on testing and I found that the cash rate at my Gyn was 65% off the regular price. My PCP has affordable cash rates and two different hospitals have been excellent at working with me as a self-pay patient. If you are worried about anything don't let lack of coverage stop you from getting answers. Talk to your providers and they might have savings programs or resources that can help you.

A few more days down the road and no changes with antibiotics. My rash is still here. It doesn't look any different, but it isn't going away. Every morning I wake up I am so hopeful for improvement. I still feel the pain. It is not constant, but comes and goes. If I get to 10 days without improvement I am going to call my dermatologist back and get back in. This weekend will be a month with this rash. I am still very scared and would like answers.
-
I would ask your dermatologist for a biopsy on the skin.
Edited to add - I'm not sure insurance would pay for it but it's worth checking.
-
Thank you, I will. Today is my 6 day on antibiotics. She expected improvement in 10 days. I am going to give it one more day and I will call in the morning and see if they can get me back in before the end of the week.
Our new insurance plan won't be active until April 1st so I have been paying for everything out of pocket thus far. Between MD Save and cash pay rates I have been able to manage. Hopefully the cash rate for this will be better as well.
I really like all of my doctors, but I feel like I have been given a lot of varied information from place to place. That part makes me really nervous. The breast center was ready to send me away for 6 months. My PCP wants another mammogram and u/s (this time on both breasts) in one month. My Gyn wants to do a f/u breast exam in 1 month. All of the above said they did not believe it was going to be anything serious and they believed if it were IBC it would be progressing rapidly and I would have another symptom aside from the rash and intermittent pain.
The dermatologist insisted that if it were IBC and I had been dealing with the rash for this amount of time that radiology would see some type of breast changes. I feel like I am having to fight my way to any answers on why I have this rash that won't go away.
The yeast cream did nothing and the antibiotics aren't clearing it up either. It still comes and goes. Last night before bed my skin looked really good and the rash seemed very light! Then I woke up this morning and the original area is back to its pink/bruised coloration. It is a small area near my nipple and it has done this many times over the weeks and even during the days. I have taken photos for my doctors of daily changes. I just want to get to someone who can give me more difinitive answers.

My PCP sent me for a right U/S today. The breast center seemed annoyed that I was back again and actually called to argue with my PCP. They had in the system that they scanned the right breast after my mammogram. They did not, only the left where the spots were seen. After having to argue they finally realized it was logged wrong and they had never done anything additional with my R breast (where the issues are).
They scanned my breast, had radiology take a look and the sonographer came back and told me all was normal and he doesn't see anything alarming and I was sent on my way. I teared up a little because still no answers and they seemed to not care at all. The mammography tech that did my tests a couple weeks ago saw me checking out and as they were sending me to medical records offered to escort me. She asked on the walk what they found and I told her nothing. She asked where I was going next or what I was doing. I told her probably back to my PCP. She said she thinks this will be nothing, but I needed to find out what was going on and not let them dismiss me. I told her about my original diagnosis as a child and how my mother was dismissed for over a year before I was diagnosed. By then my tumor was huge, kidney had to be removed and I had metastasis to other regions. She said, "good, your a pro and you know what to do". She suggested just as you have that I get back to derm and don't ask, but tell them that I want a punch biopsy because that is the only way to rule other things out.
I called my derm and due to the current medical crisis they are only seeing patients with current problems and had me in at 3 this afternoon. The dermatologist said its still there, but doesn't look to bad and its not progressing. She still believes it is not cancer related, but doesn't have any advice as to what it is or why cream or antibiotics have gotten rid of it. She said she could biopsy it, but it would leave a permanent scar. (15 major surgeries over here, my body is a scar ... I don't care). She centered up on the discolored area near my nipple and took a sample. She said it will be back from the lab in 2 weeks or less.
I am praying this is nothing and it clears up, but I am thankful I have finally gotten it biopsied and sent off.
I don't have any appointments for a couple of weeks until I see my Gyn for a f/u breast exam. Hopefully I will have biopsy results back then.
Having so many people look and nobody knowing what it is has been very odd. They all seem to agree that they think it would have progressed more or be different if it were IBC.

Last Friday I got a letter from my PCP that said they saw a simple cyst in my R breast on ultrasound. Nobody discussed with me afterward, but she said they do not suggest any follow up and it is a simple benign cyst around 9 o'clock.
This morning I got a call from my dermatologist. My biopsy came back and no cancer cells or other abnormalities were seen. She said that the only thing seen was perivascular inflamation. I asked what this meant and she just said that something was going on, but it is not cancer related. She said that she does not know what is causing the red/bruised discoloration or when it will go away. She said that I do not need any follow up and to call if it gets worse.
At this point I have had a diagnostic 3D mammogram on both breasts, ultrasound of both breasts and a punch biopsy of the discolored area. I am still on antibiotics. The only follow up they suggest from any testing is a follow up mammogram on the left breast where the two spots were found.
Nobody has ever found a cause for the sudden discoloration, increased breast size, or pain in the R breast. I have gone through all the steps and had the above testing plus blood work, liver/kidney function tests etc. Everything is normal and nothing has been found.
I guess I am done and I just have a larger right breast with a discolored spot and pain now. Nobody seems concerned and I have been to many doctors and had many tests since this began. If anything changes I will update.
If anyone thinks I should do something different or ask for somthing else I would love to know. I feel really uncomfortable that the only answer is still no answer.
-
It sounds like your doctors have been extremely thorough. Anything nasty has been ruled out, so it's probably time to get on with your life as normally as possible under our current circumstances. Wishing you the best!
-
I'm sorry you weren't able to get any definitive information.
It sounds like the doctors were able to rule out any known medical issues - try to take that as good news.
Bodies do weird things, and there are still holes in our scientific knowledge. These coming couple of months will be a tough/impossible time for getting any medical attention at all for non critical medical issues, so try and give yourself some time off worrying about it, if you can. Record any changes and pay attention if anything you do seems to help or hurt. Maybe your body will eventually be able to heal itself. Hang in there!
-
Hi the swankyhe! I just joined this forum because I too have been looking for answers for 5 months now. I'm reaching out here for advice and feedback too. I totally get your frustration and worry about not having a definitive diagnosis. Something concrete to go on.
It's the worst not knowing vs. having a diagnosis that you can accept and move forward with...or having multiple opinions from various providers.
I strongly believe I have IBC but haven't been diagnosed yet. Briefly, I'm American, 66, but live in France. Been to multiple doctors but most have no or very little knowledge re. IBC and they always say it's rare and that I don't have it. In spite of sharing qualified medical information from US Cancer Centers, I still get treated like I'm crazy.
I've had most symptoms of IBC starting last January: pinkish/redness covering portion of right breast, somewhat swollen, and horrible shooting burning and stabbing-like pains. Over a short period of time it then spread to other breast. The discoloration comes and goes. Of course it never presents when I'm in a Dr's office so I show pictures on my phone!
All lab tests have shown nothing abnormal. I have no lumps, tumors or masses. I almost wish I did!
My situation is bit more complicated in that I had had implants for many years. So I sought to have them replaced and capsules (scar tissue) removed also in March just prior to Covid-19 lockdown. My surgeon and I felt this should've relieved me of all the symptoms. But no! Five days post surgery the distinctive pains and discoloration returned. Then I know it was something systemic.
I have an appt with a Dermatologist Monday for a skin biopsy--provided she does it. My surgeon who ordered it says she may not do it she doesn't see anything abnormal. I'm praying she will do it. It's the only way to confirm or rule-out IBC. I just want to know!
And an appointment for an MRI and with an Oncologist mid-June. Requested mid-April during Covid so very long wait times.
I do wish you well and hope you have resolved your own personal situation by now.
-
Hi there! That is very scary and I am sorry that you are also struggling to get answers. I still have a discolored area on my R breast that comes and goes and have the burning pain that comes and goes as well. I had a follow up with my Gyn 2 weeks ago for a repeat breast check and he does not feel anything or see any new discoloration. He is not concerned. He did send me for a repeat mammogram to check for any change on the spots on the left side. He didn't want to wait 6 months and I didn't either. Nobody has an explanation for the issues, but all say I have nothing to worry about. I too have breast implants (textured saline) that are almost 13 years old. After my punch biopsy, lab work, and imaging all came back clean I was told to consult with plastics. I had expressed that I wanted my implants removed and my PCP also felt like this would help with my issues. I had my first consult with my plastic surgeon via teledoc due to Covid. I have since been to see her and we have a plan for implant removal and capsulectomy. She said that she doesn't believe my skin discoloration is anything to be afraid of, but she will be performing a lift as well and will remove the section of skin/tissue for pathology. I have already had the punch biopsy, but she said she would like to do this to be safe and I am sure to give my piece of mind. I was able to talk to her about my medical history and all of my concerns and she says that anything she sees that looks abnormal while she is there she will remove for pathology. Were they able to do any testing on tissue or fluid during your implant removal? It is scary and more unnerving to talk to different professionals and be told conflicting things. I was told by the first doctor I saw that if there is no lump there is no need to worry (we know that is not true) and told by another that IBC is always seen on a mammogram (also not true). It is scary to not only have a lack of information, but to hear things that you know are not true from people that are treating you. I am glad they are continuing to push for answers for you. I feel good and I really feel more at ease that I don't have cancer again. I just would like to know why my skin changed overnight and has never completely cleared. Please let me know when you are able to see an oncologist and I hope they find answers for you!

jlk
If you've had it for five months, it's very unlikely to be IBC, which spreads very rapidly without treatment.
-
To my knowledge IBC does not spread to theother breast either.
-
Please don't take this wrong, but I would not be surprised if your issues are due to implants. I've read so many posts where people have many different types of issues with them. Also, I see a lot of people on here who are convinced they have IBC, and rarely do they actually end up having it. At this point if you did have IBC the doctors would likely be able to detect it.
