Where do peri menopausal women fit?
Hello all,
I’m wondering if anyone can answer my question. I was diagnosed in August and turned 51 in September. I was having my periods but was also taking oral contraceptives. Because the cancer was hormone receptor positive I immediately stopped and had my last period end of August. I have not had one since. Nurse said I was probably now perimenopaudal I have been looking a guidelines for treatment for my specific pathology under 50 premenopausal has chemo suggested and over 50 post menopausal chemo is not. I have grade 2 idc and grade 3 dcis, lvi present, 2.1 cm estrogen and progesterone positive and her2 negative and negative node. I am wondering which classification I would be in and which grade they would consider when deciding treatment. Any ideas? Thank you.

Did you have an Oncotype test done? With the Oncotype results, only scores in the 20s range sometimes have different recommendations for '50 and younger' versus 'over 50'. So if your score is not in that middle range, the recommendation would be the same regardless of age.
As for where you fall, given that you still have periods, I think most Oncologists would conservatively put you into the 50 and under group. And you were in fact 50 when diagnosed and certainly less than that when the cancer developed.
As for which grade is considered, treatment protocol is based on the invasive cancer. When someone has both DCIS and IDC (as about 80% of people with IDC do), the DCIS is considered incidental and once it is surgically removed, anything else done to address the invasive cancer is considered sufficient to address the DCIS.

If chemo is recommended it will most likely stop your period and by the time chemo and radiation therapy are done you might be considered post-menopausal. Without chemo I don't think they would consider you in menopause because it's been less than a year from your last period.

I have not had Oncotype test done but was reading the clinical guidelines from my region trying to figure out whether they will offer chemo my idc dcis tumour is grade 2/3 but has lvi positive which in this publication is considered high risk , but that is only high risk feature

Re: still having your periods and oral contraceptives. When was about to turn 50, my GYN advised me to stop oral contraceptives and then test my hormones to see where I was hormonally. I had been having regular periods while taking oral contraceptives. Three weeks after stopping, my blood was tested. I was fully menopausal and never had a period again. Being on the oral contraceptives acted like HRT in a way and I had no idea that I went through menopause. It was quite a surprise to everyone!
