Mother- I had a rough recon. Not because it was sub-pec but because my right side had too much tissue taken during mastectomy. I never had issues with tightness and deformity that others describe so I felt the added layer of natural protection to implant and ease of examining breasts was on my side. Quite frankly my PS will do either, but at beginning was pushing sub pec. I didn’t think further.
My PS wasn’t a fan of all the Alloderm that’s involved in pre-pec reconstruction, not sure why she was all uppity on that. I’m going to ask her that today, as I have a long time with her to wrap my head around this.

Hi Motheroftwo. I was really worried about nipple loss until I saw the videos on YouTube about the Healing Tattoos of Vinnie Myers. That made me feel a lot better. I had surgery last week. I will be thinking about you next week!

I think I am planning on over the pec muscle - has anyone had this with the non-textured implants? I seem to find women that are happy with it, but they have the textured implants, which seem to hold shape much better. I fear the pre-pec with the saline implant will make me feel like I have floppy water balloons on my chest...am I right?

My PS cautioned me against saline when doing pre pec placement. She said you would feel sloshy (her term). I have smooth (thank god) gummy silicone implants but she had to use alloderm to make a pocket sling to hold it. My implant placement is slightly modified pre pec. I think the results are very good. Wish they felt as good as they look but oh well.

@Lexica...I had a prepec, and my PS only uses smooth implants. I have smooth rounds made out of silicone. My choice to use silicone, but I get the impression that the vast majority of his implant patients make the same choice now. No "sloshiness" but I do have minor rippling (but not enough for me to undergo fat grafting.)

I had pre pec smooth round silicone. I had rippling as well but did have fat grafting and it’s much better.

Lexica I do have some PMPS. Overall not too bad and definitely improving with massage therapy and working out with small weights. I think a lot of my difficulty is where the PS did a "sub plane" technique where just a small part of pec muscle is stretched over the leading edge of the implants at the center of my chest . It covers maybe 1/16 of the implant so that the transition would be nicer. Thats where I feel the most - I don't want to say discomfort but awareness maybe?

Hi ladies,
I chose the pre-pec but right now only have my spacers. I have surgery planned for June for my perms which will be silicone. I thankfully have a PS that mainly does pre-pec and did a wonderful job. Sadly at this time i have the funky look where you call clearly tell where my implants are and the edges feel very weird. They are filled with saline but I don't get the sloshy feeling.
One aspect i do like about the pre-pec is the slight sag. To me they don't look real yet... waiting for the perms with fat grating, but no women's breasts are always perky and without sag, so it helps me at least feel like they are more normal.
I chose to have my nipples removed as the chance of the nipples possibly having issues where they don't get enough blood flow and have to be removed anyway did not sit well with me.
I wish all you ladies faced with this decision the best of luck. I agree with the ladies that have suggested another opinion. This is your body and your choice, you shouldn't be pushed to choose what you might not want.

I agree, I had a revision from subpectoral to prepectoral and besides the lack of animation deformity, I really like having a little bit of, as you call it "sag". It's much more normal. Super perky is ok when you are 17.

I choose over the muscle and I'm extremely happy with the results so far. My plastic surgeon recommended it and said it will produce the most natural looking result. I was able to do nipple sparring with no issues.
I still have expanders in, but even with expanders the placement looks really good. Since my the muscle was not cut into, I didn't have any issues with range of motion once my drains were out. I had almost not pain after surgery. I took Tylenol after surgery mostly bc of the drains. At four weeks, I'm cleared to work out doing everything except planks and pushups. I can restart those in two weeks (six weeks post surgery).
I have to do radiation, so I won't be getting my actual implants until 7-8 months from now. I will be going with silicone. My surgeon, like Rah2464 's was also against saline for over the muscle.

I had my prepectoral implant surgery on July 15 - no expanders, as I had a skin sparing mastectomy. They are a bit small, but that's what I wanted after have too large breasts for most of my life. The results are okay - my only issue is the size and that my mastectomy scars (which are wide and horizontal across the middle of my breasts) have made them a bit wonky. I was hoping that fat grafting might fix that, and maybe add a bit more depth? Not sure if that is possible since the scars are pretty inflexible. I have my post op appointment in a week, so I hope to discuss it then.
When I came out of anesthesia, my temperature dropped to 95.7 and I had to be treated (warmed) and observed overnight for hypothermia. That makes me a bit nervous about going under again. My BP was pretty low, too. I felt out of it for a week after. I had no complications after my mastectomy though - none at all. Anyone heard of this before?

Thought I would chime in...I have sub pec with lots of fat grafting, incrementally, over several surgeries. My results are amazing, actually. My PS and I discussed pre vs sub and told me that he would best make the call during the surgery. After 2.5 years....he has never lead me wrong

I am just adding my experience for folks doing research. I had direct to implant above the muscle reconstruction after a unilateral left mastectomy on May 9. My cancer breast was a full cup size larger than my healthy breast, and my PS was able to match sizes pretty well. Recovery from surgery was great--I was back at work part time in 2 weeks. I only needed a couple of Tramadol for pain. I am back in yoga doing planks and down dogs without problems (although I have still not regained all my strength..my PS made me sit out of yoga for 6 weeks postop).
Admittedly, absolute symmetry and aesthetic perfection was less important to me than ease of recovery, but so far I am EXTREMELY pleased with the cosmetic results. The scar is jarring, of course, and I miss having a nipple (nipple sparing wasn't an option for me). I will get a 3d nipple tattoo sometime next year. I haven't noticed any rippling (and, to be fair the rippling would have to be VERY noticeable for it to be worth going in for more surgery to me).

I don't think you have to be put under for lipo and fat grafting. I'm having a Nipple-Sparing Bilateral Mastectomy with Prepectoral Implants on August 19th, and I will have to do lipo/fat grafting as the second phase of my reconstruction. I will be a C Cup. What size are you?

Hey Girls- Have any of you had experience with previous mastopexy with under the muscle implants prior to BMX with reconstruction? Tissue expanders are under the muscle now. Silicone implants will be placed latter under the muscle. How do you look in clothes is my big concern. Many thanks
