left nipple itchy concern ( last almost 2 months already)
Good day everyone,
I have some concerns about my left nipple and hope if someone could give some suggestions.
I live in Montreal and I'm 39 years old.
my left nipple starts to itch in early Jan. 2019 and I noticed a piece of dry skin peeling off from the nipple. I put some Avon eczema cream on it, but it won't stop itchy. and I also noticed yellow liquid discharge from that nipple and some left on the bra. so I went to the walking clinic to see the GP for the 1st time at end of Jan. 2019
the GP prescribed me two medications for 10 days : Hyderm 1% cream and Bactroban cream
He also ordered ultrasound for the breast and mammography for me
I put these two creams for 10 days, the itchiness in the nipple was getting better, then repeat itchy again.
I went for two scans as well.
since the itchiness in the left nipple doesn't getting better, also I want to know the results of imaging. so I came back to the clinic for the follow-ups. this time I saw another GP.
She said had checked my image result and there is nothing wrong shown for the left breast.
I talked with her for my concern for a rare type of nipple disease and asking whether I could see a breast specialist and for a biopsy.
she concluded I only have the skin infection and prescribed me another antibiotic cream: Clotrimazole for a week.
She didn't give me the referral for breast specialist but recommended me to see a dermatologist.
Actually the antibiotic cream she prescribed doesn't help mush to stop itchiness.
I contacted the dermatologist for the appointment in 3 wks, that's the fast appointment I could get.
but for now, my left nipple continue to itchy, also notice the left nipple skin color becomes lighter than the right one, I also feel discomfort for the left breast
I look through internet and have some concern regarding a type of nipple cancer disease called paget's disease in the nipple.
I wish it's just me overthinking about my symptoms, but I really don't want to wait to have clinical symptom appears then going back to run the test, that maybe too late.
my concern is since there is no tumor show on the image of ultrasound and mammography, the doctor won't let me go further.
I would rather to have the breast specialist to have a look and I'm willing to do the biopsy just to be sure.
Could someone provide some suggestions to help me

mtl114 - The only way to test for Pagets is with a punch biopsy. My scans all came back negative as well so my GYN referred me to a dermatologist. I did the prescription steroids as well for 10 days but they did not help. It was the dermatologist that did the punch biopsy. Your symptoms sound very similar to mine. Is there any way for you to get in sooner with another dermatologist?

can I insist on the biopsy or it depends on the dermatologist? I really don't want to apply the 4th cream on the nipple, these medication just make this part so sensitive.
from the experience I saw the GPs here in MTL, they just concluded the breast cancer should always related to the lumps under, or there is nothing wrong.
I have the impression since the paget disease is so rare, it seems not all the GP even hear about this disease... felt so discouraged.

I think you may be able to get in sooner to a dermatologist since all you're looking for right now is the punch biopsy. I agree, that Pagets isn't seen as often but it is possible to have Pagets without any other underlying issue. Therefore there would be no lump to feel. In my case the only thing that picked up the DCIS under the Pagets was the MRI after the Pagets diagnosis. Since you've already tried a prescription ointment I don't see any reason whey they wouldn't just go forward with the biopsy.

I went to see my gyn yesterday, but he said didn't see the nipple, but he gave me the referral to a general surgeon to check.
I still wait for my appointmt with the dermatologist to see if he could do a biopsy.
I also went directly to the breast clinic in the hospital, but the receptionist told me as below:
here breast clinic is a cancer center, if my mammography looks fine, then I can't be there
I actually just want try to have a breast specialist for the consultation.
I checked your previous post, it's sad to know you have the peget's result positive.
I also saw you mentioned yours are perfectly normal but peeling often.
what type of peeling you have, it's the huge part of skin or just minor dried skin?
I noticed mine is dry as well now, still itchy and also sensitive, harder than the other, the color also lighter than the other

I've been on the fence about posting on this forum but my symptoms have been the same as yours! The only difference is that I haven't experienced any sort of discharge.
My PCP put me on clotrimazole and I had normal imaging results as well, though my blood results noted a high amount of prolactin so I have to follow up on that at the end of the month because I’m not pregnant. Clotrimazole stopped the itching for a bit but the flaking and itching came back when I stopped using it after a few weeks. I also started developing stabbing pain all over my breast, shoulder blade and arm, so I went to a breast clinic for a second opinion. The doctor said he could feel the difference between my breasts but thinks it's likely just a skin problem and wants me to continue taking the cream given to me for two weeks before seeing me again. If the problem persists he'll refer me to a dermatologist to rule out Pagets. So back to waiting!
This whole ordeal has been really stressful and I truly hope it's just a weird skin issue of some sort but I've never had any skin problems besides regular acne so I can't help but expect the worst. On top of all of this, my appetite has grown extremely spotty and I've developed a persistent dry cough so my brain is absolutely haywire with paranoia these days.

thanks for the post, I just noticed I did have the blood test request, but I total forget to do it. I will do that tomorrow morning in the clinic to see if have the same result as yours.
for now,I'm just running over here and there to see could have a real specialist to check my situation.
here in MTL, almost 99% only believe the image result, if it's negative, means nothing matters.
I feel huge frustration as there are other pbs which also matters but won't show on the image.
we all hope it may be us over thinking, but we need a proof to keep us peace mind, now I'm still searching for the help.

My peeling was very minimal, just the nipple. Again, nothing showed on my imaging so don't let your clear images lead you to not pursue. A dermatologist can do the punch biopsy and if anything will set your mind at ease.

Agree with MBPooch 100% - you need to go to a breast clinic and ask for a punch biopsy. I can categorically state that Paget's certainly does not always present with an underlying lump, and even if there is something underlying it won't necessarily be seen on mammogram / ultrasound / MRI. I am proof of that, I had obvious Paget's open sore on nipple and slight discharge, sensitive /itchy and painful. NOTHING underlying showed on mammogram or ulstrasound, but the breast surgeon still did a punch biopsy to test for Paget's. it was positive so I had the nipple and section of tissue underneath removed. When the tissue underneath was tested they found 2cm DCIS high grade.
Due to the fact that there could be other areas lurking unseen by mammogram and ultrasound they offered a mastectomy. I am having the op next week.
Keep insisting. Good luck.

here in MTL, my case gets difficult.
I saw 3 GP.s, none of them think it matters, only skin issue, even I show the paper I print out for paget's disease.
I went to breast clinic but they refused to serve since the image is ok.
I will check with a general surgeon next wk and will try to have the dermatologist appointment next wk as well
btw, where could get appointment for breast specialist or breast surgon?

Would it be easier to focus on finding a dermatologist right now? They can do the punch biopsy and you may be able to get in and have it done sooner than trying to get into a breast specialist.
Eliza - my situation was SO much like yours! High grade DCIS underneath the Pagets that did not show up on mammogram or ultra sound. It did show up on MRI after my Pagets diagnosis. I also did a BMX and haven't regretted it for a minute! Recovery was also not nearly as bad as I had imagined. Good luck to you next week!!!

I am in the UK. My doctor sent me straight down to the breast clinic at our local hospital when I first went to visit him with sore. I ven have Psoriasis so I was very lucky that he didn't just think it was that, as has happened with so many people when docs think it is just psoriasis or eczema and send them away for 6 months with just creams. I cannot thank him enough for his diligence.

It was your story originally that helped me decide on the mastectomy decision if you remember, after my lumpectomy for Paget's and then the DCIS diagnosis, my options were mastectomy or yearly screening. It was a no brainer but helped by your and others experiences. I did ask for a BMX for symmetry, as I never wear a bra, but they were not happy with that, however they have said to wait for 6 months or so after the MMX and if I was really not happy living with the one, then they would consider a 2nd mastectomy for symmetry reasons as I am so averse to reconstruction.

I’m in the US and needed a dermatologist to do a punch biopsy and test for Paget’s. Mammogram and ultrasound showed nothing. When the biopsy test came back positive, the dermatologist referred me to a breast center. Do not worry if the earliest appointment is in a few weeks. If you can get it sooner, great, but 3 weeks isn’t too bad. They will have the results about a week after. Yes, you may need to insist on a biopsy, which involves taking a chunk of skin off your nipple. The pain medication shot stings, but the biopsy itself shouldn’t hurt. Maybe take Tylenol beforehand to help once the shot wears off. If you get a positive diagnosis of Paget’s, a breast center should get you in more quickly (here it’s about a week) and start treatment.

Try to get a sooner appt with Dermatologist. The discharge is what's concerning.
I had a scab form over my nipple after radiation. I picked it off and it came back. I was also concerned it might be Pagets. My RO gave me Triamcinolone cream and I used it for a month and went back. It was better. Also had mammogram with good results. She did not think it was cancer. Neither did my MO. But both of them asked me if I had discharge and I did not. If I had, I feel like they would have sent me for a biopsy at Dermatologist.
I was told Pagets moves fast, so, if you see changes, call your doctor and insist they see you again. Good luck. I know the fear you are feeling, but the Dermatologist is the next step.

I only had a uni-mx, and I too had a lot of difficulty making the decision. One of the things that made it so difficult is that I was expecting that if I made the "right decision," I would feel better about things. At some point, I realized I did not want to make either choice - I just wanted to be well and have a normal body again! - but I needed to make a decision and move forward, for my health. It removed the pressure to "feel good" about the decision.
I thought about what was most important to me - healing fast, getting back to my life, not having more surgeries if I could avoid them. I also know I get very distracted by changes in my bodily sensations and it seemed like the implant process would be distracting for me. I felt more confident about my ability to control my shape with forms/stuffing, than a more fixed implant. One other bonus of the non-recon option was a faster surgery. I'd had two surgeries already and was so tired and needed to be done.
As you can see, it's a very personal decision. I know women who were willing to do whatever it took with their recon because they wanted a shape like their pre-diagnosis shape. I was not as attached. I felt like if I had any complications or needed more surgery, I would regret my decision.
We all have tough days when we think "what if I'd taken the other path" but in the end, both are acceptable options and we just have to pick the path we're most willing to take forward.
Know that you may not feel great about the options you have, but you are taking your best shot at removing the cancer and that is the most important thing. The rest you will figure out - women live good lives both ways.

Hello everyone, thanks all your help and suggestions.
I went to see the dermatologist yesterday, who has a very good reputation in MTL.
he checked and said it doesn't look like paget's disease, he said my left nipple only a bit swollen which is caused by eczema,
He prescribed another cortisone 0.1% cream to apply once per day if it itches.
I knew I have to insist for biopsy, I didn't get a chance.
if my situation doesn't get better in a wk, I will come back to see him and ask for a biopsy or I could change to see another dermatologist for a 2nd opinion.

Glad you're going to stay persistent! It could very well be nothing but mine didn't "look" like Pagets either.

I have had intense itching in my left nipple also. It is driving me crazy. I don’t have any rash or open sores. My skin looks completely normal. I’ve tried over the counter cortisone creams but it doesn’t seem to help. Did your symptoms ever resolve? Did the prescribed cream help?

I suppose I was lucky. I told my dermatologist about being a survivor of IDC in other breast & had a rash on other nipple. She said she could give me a cream or a punch biopsy. I opted for the biopsy. I don’t have results yet but feel relieved I got test done.
