Two types of cancer in one breast

Dear Community:

Just last week, 2 nodules were detected in my right breast. First nodule is lobular invasive carcinoma 1.2 cm, second nodule is Ductal invasive HR2+ of only 5.8 mm. Via MRI, Ultra sound and biopsy of one slightly enlarged lymph nodes under my arms they did not detected any affected lymph nodes. So that part came out negative. I will have surgery, masectomy of one breast this coming Thursday.

My question is, has anyone been in familiar situation? If they see no lymph nodes affected, will they still recommend chemo even if my Ductal hr2+ is so small? And radiation?

They said that so far they don’t know until post surgery and when the pathology comes. Only part they are sure of so far is the hormonal therapy.

Also, I want to add that 4 years ago I had papillary thyroid carcinoma. All ok there now!

    I had 2 tumors on left side, one ilc 1cm and one idc with lobularfeatures 1cm. I had no nodes involved. There was 4cm separating the tumors. Tumor close to surface, but no skin involved. It was thought multifocal multicentric after mastectomy it was determine with 95% confidence they were 2 synchronous tumors separate occurrences. Which was good news one was grade 1 snd the other was grade 2. Both had mitotic score of 1, slow dividing. My her2 was equivocal but I thought it was equivocal after FSH test but it was below threshold so her2 negative. My er was 95% pr was zero. My oncodx was 34, but I didn't do chemo I did 4 years AI therapy. Still here no cancer, 7th year.

    I am just wondering, prior to surgery did they do a centinela lymph nodes prep so to confirm during surgery that no lymph nodes are affected?

    In my case, via imaging; biopsy of one auxiliary lymph nodes (it was a bit inflamated) , breast MRI and a few detailed ultra sounds they say no lymph nodes are seen affected and the chances of it not be so ( via the centinela test during surgery) is only 10%.

    Just wondering your intake on it. I guess I am a bit nervous since my surgery is only three days away 😊

    Dela, yes that is what I had the MRI gave me a 10% chance of finding cancer in lymph nodes. Then during the mastectomy they did a sentinel node biopsy 3 nodes and showed zero out of 3 no cancer. The sizes of tumors were the same as what MRI had shown, and suspicious ultrasound area turned out to be normsl tissue.

    Good luck on surgery, they drain was not uncomfortable like I thought the plastic is very flexible.

    I had DCIS and IDC in my right breast of this year. 20 plus years ago I had DCIS in my left breast. I had 2 surgeries as now I realize that was because margins were not clear. Recovery was a breeze. Never even left a scar. I had 3 tumors this year and 1 lymph node involvement. I did not know prior to surgery if there was lymph node involvement. I wanted a lumpectomy, as before, but 3 tumors spaced out a bit made that impossible. During surgery they checked sentinel node and found cancer. They then removed tissue under my arm taking out an additional 11 nodes. I am still 3 mos out from surgery doing PT due to hyper sensitivity in my right arm and breast area and swelling. I would highly recommend PT. The drains were no big deal and neither was removal. However due to hypersensitivity I have been bound at the chest and arm until a week ago. That is my story

    redgirl, glad PT is working. I found relief in swimming laps, 1 mile but after I get home maybe 1 hour or 2 later I feel the tightness again. It has been 6.5 years since surgery.

    What can really whack me out is car riding. It is the bouncing that eventually makes my arm get tight, stabbing pain, and hypersensitive. I must massage it like a baby to assure my arm everything is ok. Also raise up the arm. Massage especially under the armpit. My chest is the same. I just can get so sore. Today I was on a day trip. Whew. Sore.

    My situation was similar to Meow with both lobular and ductal tumors. The micromets in my sentinel node were not seen during the quick check during surgery, only after complete pathology was done. Since that was 7 years ago, complete axillary dissection was performed plus I had chemo. No radiation because it would have compromised my reconstruction. Has anyone suggested genetic counseling?

    Now I have lymphedema - I hope redgirl has a physical therapist trained in treating LE. Mine conditionis stable using a regimen of water exercise and my Flexi Touch machine.

    For Dela, it sounds like you are well prepared and will do fine. My niece spent last year in Spain during college. Wish we could have gone to visit while she was there. She just graduated and is going off to Japan!

    Have you heard of the surgery implanting lymph nodes from another area? It is treatment for lymphedema. I heard a BCO members story and her good experience with it. But don't know if it would be done for a mild case.

    My PT person had bc herself - twice. She found out she carries the gene. She specializes in bc. However, nobody can be certain lymphedema will not occur early on or later on. I try to listen to my body. I protect that arm. My sensitivity was all the way to the wrist right after surgery. Awful. Felt like knives and electro shock every few seconds. I wore sleeves all the time. Today the sensitivity is pretty much just around my armpit. A bit on the back of my arm. I do daily exercises several times a day. All I can say is it is getting better. So the trend is good.

    Hi, I am 37, and I'm two years out from papillary thyroid cancer and am two cycles into my neoadjuvant chemo regimen for my breast cancers. I have a Grade 3 TNBC (5cm) and a Grade 2 ER+ PR+ HER2- (3.5 cm). They are in the same breast and in the same quadrant even! It is confirmed in my nodes and possibly in my sternum but they can't biopsy it to confirm. So I'm stage 3, possibly but hopefully not stage 4. We are pretty similar in having had thyroid cancer before so that's interesting! How are you doing? Hope you are doing well!

    Lumpectomy 10/15/18, Solid papillary carcinoma with associated focus of invasive ductal carcinoma. Grade 2.


    Finding the invasive prompted the surgeon to order an Oncotype on the papillary which came back with an unexpectedly high score of 34.


    So now Chemo is in my future

    I too have been diagnosed with 2 types DCIS and Invasive Mammary Carcinoma. The tumor was Stage 1 Invasive and the surrounding tissue was DCIS. I am currently waiting for my Oncotype number. Did you have a family history or test positive for BRCA? When do you start chemo?

    Ann Mar~ I have one elderly aunt who had BC but no other family history of it. I don't believe I was tested for the BRCA gene.

    I get my port installed December 19th and will start chemo in January of 2019

    (I asked for a delay to travel out of state to celebrate my mom's birthday)

    I just had a secondary cancer as invasive papillary carcinoma (stage 1) in left breast from pathology test on Dec 7, 2020. I was diagnosed with breast cancer on 05/27/2014 with er+, pr+, her-2/neu negative for Left breast upper inner infiltrating ductal Carcinoma...I am not sure if I should chose single or double mastectomy. Any experience and suggestions would be very helpful! Thanks for your support!

    qins, I suggest you start a fresh thread of your own in the JUST DIAGNOSED section.

    This thread is old and was more about having 2 different cancers at same time, not many years apart.

    I think you'll get more responses.
