BI-RAD 4 w/ internal vascularity

The waiting may kill me! I had my mammogram on November 15, followed by an ultrasound and a biopsy scheduled for December 4. Here are the results of my ultrasound --- can anyone give me any "predictions"? I've been searching and find lots of various outcomes. Thanks. I'm trying to stay positive, just very anxious to know what is going on with my body.

EXAM: LIMITED ULTRASOUND RIGHT BREAST.

INDICATION: Call back for density seen in the screening mammogram.

FINDINGS: Focused ultrasound of the outer right breast is performed. At 9:00 right breast 11 cm from the nipple, a 11 x 5 x 5 mm hypoechoic lesion is noted with microlobulation and minimal internal vascularity. This is indeterminate and tissue
acquisition is recommended to further evaluate. Findings were discussed in detail with the patient and patient is in agreement for needle biopsy for diagnosis.

Findings are notified to nurse, Salone at Dr. Loftus office and verbal referral was obtained

ASSESSMENT: BI-RADS 4 Suspicious.

    With that description it could go either way. There is no mention of "acoustic shadowing" (which tends towards malignancy) or "good through transmission" (which leans towards benign) which to me are very important features to describe in a report. Benign things can be vascular so thats not really helpful except to say cancers can be very vascular. Indeterminate is the correct term for the finding.

    If it is a cancer it measures relatively small in the scheme of things. Was there a study from last year to which it was compared or was this your first mammo?

    Here is the mammo from last May, it doesn't show anything.

    EXAM: BILATERAL DIGITAL SCREENING MAMMOGRAMS WITH TOMOSYNTHESIS

    COMPARISON: 10/1/2015, 8/14/2014.

    HISTORY: No first degree relative with breast cancer.

    FINDINGS: Breast tissue is with scattered fibroglandular densities.

    CC and MLO views of both breasts are obtained along with tomosynthesis.

    No suspicious microcalcifications, spiculated density, or focal architectural distortion seen in either side. Overall, no significant interval change seen in either side since the previous comparison mammograms.

    Analyzed by R2 CAD.

    ASSESSMENT: BI-RADS - 1 Negative.

    RECOMMENDATION: Annual screening mammography for both breasts.

    Not a doctor myself, just a fellow breast worrier here (back in June-July).  I'm sure that djmammo who's a certified radiologist and active member in our forums here will soon respond to your questions.  I can only share the laywomen's common knowledge regarding imaging findings.  Birads 4 findings have 80% benign 20% malignancy outcome odds after further imaging, biopsy and/or lumpectomy.  Your finding is wider-than-taller which generally is a very favorable thing.  Furthermore it's only microlobulated and not spiculated, ill-defined, indinstinct or obscured which again is a good thing.  Birads 4 assessment means that the finding is indeterminate through imaging and some sort of biopsy must be conducted so that a definite conclusion can be drawn.  Retrospectively when reflecting on my own summer experience with birads 4 finding and the consequent course of action (mri and other contrast imaging, lumpectomy and final histology) I'd say that instead of letting anxiety get the best of you, just keep your cool and stay focused on swiftly pursuing the next steps (needle biopsy, etc) so as to regain your peace of mind the soonest possible.  Sending tons of benign wishes your way, please don't let the waiting depress you or deprive you of proper nutrition and sleep hours <3

    Was this something new you felt that prompted the recent exam? From the May screening report I assume you felt no lump at that time.

    I didn't feel the lump. I was actually looking into having a breast reduction and was about 6 months overdue for my yearly mammogram, so that is why it was scheduled. They discovered it in my mammogram, then I had the ultrasound.

    One more day until the biopsy, wish I could get the results the same day! I've been trying to stay busy so as not to focus too much on this. But, must admit I am pretty nervous.
