Biopsy results in and still questions

My biopsy results say atypical ductal hyperplasia, radial scar, PASH, and some other things. Then it states that sample size was insufficient to rule out DCIS or other cancer. I knew I would be having an excisional biopsy/lumpectomy anyway but now I’m frustrated because there is still a possibility of cancer. Has anyone had this happen and after lumpectomy they were upgraded to DCIS or another breast cancer? I was sooooo hoping to be done with the wondering.

    puzzled, in about 20% of cases of ADH there is an upgrade to DCIS (or invasive cancer) on surgical biopsy.

    You're welcome! Where are you?

    The stats are about the same on upgrades from ALH/LCIS too, but I had nothing worse on my excision.

    Hello puzzled1

    My biopsy also show ALH mild to moderate and columnar cell hyperplasia. It appears we are in the same boat. Sigh. I have an appt. with a breast surgeon 10/29 to discuss pathology. I am not sure if we will wait/watch or do an excision/lumpectomy biopsy. I am also scared of an upgrade.

    Hey, Cas! Sorry you’re in the middle of this too. I saw my breast surgeon yesterday and I’m having the lumpectomy next week. Really not a lump there so I get the lucky wire put in my breast to guide her. Yuck.Waiting to find out if it’s a Tuesday or Thursday. She said just watching and and waiting wasn’t an option . I’m high risk anyway for many reasons so that maybe why she doesn’t recommend waiting but my research shows radial scar or ADH is almost always removed. Better safe than sorry! Good luck!

    Good luck on your upcoming surgery. I also saw my BS today and I will also be getting a wire guided biopsy. No date scheduled as of yet. I am hoping B9 results for you and no upgrade. She told me 5-10% could be upgraded to BC. So fingers crossed for us both. I was told 7 days for pathology results. That will be another hard/LONG wait!

    Hi Cas! Glad you got to see your surgeon. Lucky us with the wire and surgery. Mine is this Friday. She said I would know in three days and 20 to 30 percent chance of finding something a cancer. We are hoping no surprises though. I pray that yours goes well and everything is good. ❤️

    I like the 5-10% upgrade chance better. ;) I know MelissaDallas stated above 20% for both ADH and ALH. I guess just be prepared and be relieved if it doesn't upgrade. Good luck on Friday!

    CasM, I have not yet scheduled my excisional surgery. I had LCIS and ADH results from core needle biopsy. I have a full calendar at the moment, was told that's fine & no need to cancel plans, just can't put it off more than 2 months :) I admit I worry about upgrade but my surgeon has not been panicked so I'm taking my cues from him. I do understand the stress between now and then can be much. Been there done that waiting on biopsy result. I can't function like that so I'm keeping faith on the favorable odds. I was told there's a 75-80% chance to find result without upgrade. 🙋

    The only thing I'm worried about is being awake. I thought it'd be under general anesthesia but my surgeon said it's not 😓 Saving my big girl panties for that :)

    Hi Snowchick, thanks for asking. It’s been a little rougher than expected. Was hoping the pain would be like the mri guided and stereotactic biopsies but nope. I thought I was going to have twilight sleep too but when I got there it was changed because the area was bigger than expected. What you’re probably having will give you amnesia for the procedure and you won’t remember it - king of like during a colonoscopy. I know lots of people who have done that for a lumpectomy and said it was just fine. Good luck on yours

    Yes, I was told wire excision I would be sedated but not under general anesthesia. My biopsies were excruciating so Im leary to be awake for any part of the next step. This is not my first rodeo, I had core biopsies and wire excision over 10 yrs ago. Different hospital, surgeon & circumstance-- wide awake and no problems. This time around everything has been very painful.

    Puzzled - so sorry to hear that the procedure was rougher than expected. That is NO FUN!! I hope you are feeling better today and got to rest a lot over the weekend.

    Snowchick - I didn't feel a sense of urgency from my breast surgeon either. I was hoping to get it completed quicker just for peace of mind. I like your thoughts on the 75-80% chance of no upgrade. ;o)

    CasM, I understand about waiting & peace of mind. Maybe scheduling could add you to a cancellation list so you might get in sooner? ✌

    Thank you, Cas and Snowchick, today is definitely better. Most of the swelling is gone and pain is minimal. It will just be black for a while. Y’all probably wo t have the problems I’ve had. I am old and have a chronic illness.I pray that both of you are able to have yours done soon with good results. I am supposed to get my results today

    Cas, I saw on another thread worries about the wire localization procedure. I was more worried about that than the surgery, lol! I didn’t need to be. They definitely numbed mine and there was only a sting for the needle numbing and a little discomfort with the positioning. They did take lots of pictures and reposition the needle a couple of times but there wasn’t pain because of the numbing. I was sitting up but I know that some places do this lying down. I wouldn’t worry about this procedure if I was you. You will sail right through it.

    puzzled1 - so glad to hear that you are doing better and I hope your results are favorable. Thanks for the encouragement on the wire placement. It is definitely my biggest fear and I am happy to hear that part of your procedure wasn't painful for you.

    Praise the Lord, no cancer! She took a large area with clean margins. Said I had a lot of stuff going on but no cancer. Praying for results for yall
