Question about tissue expanders

Hi!

I am having a bilateral mastectomy August 29th. At this juncture I am to have skin/nipple sparing with tissue expanders and then pre pectoral implants. (I had radiation on my left breast 7 years ago and am not a candidate for direct to implant) I am wondering.....if it is skin sparing, why are the tissue expanders necessary? Won't there be extra skin already available after all the breast tissue is removed? I am a DD. Maybe they are just there as place holders while I heal? I will have to ask my PS. This waiting is killer. <3

    Druanne yes they are there for 1.space holders while you heal 2. They do not encourage having radiation done on implants as the possibility of ruining the implants is high with radiation which i am guessing you will be getting. I hope this helps ease your mind. Best of luck

    Princess_Meg Thank you so much! Yes this does help to ease my mind. As of right now I am not having radiation as it is only LCIS in my right breast and nothing in my left (THIS time) If they end up finding any cancer though I am sure that will all change. That makes sense as a place holder. So when I wake up I will not be flat? Some days I feel really ok about all of this and then there are times I am filled with anxiety. Thanks again for answering me. :)

    Druanne, I had a nipple/skin sparring BMX on 6/4 and my PS used TEs coved with alloderm over my muscles. The alloderm holds the implant (or TEs) in place, and helps make the skin thicker so you see less rippling. He told me he uses deflated TEs to allow slack in skin so the incision healed properly, and because it gives a better cosmetic outcome. I think the tissues can begin swelling during the breast tissue removal process, so this allows the PS to see how everything heals and then he/she can revise the 'pocket' at the exchange too.

    The deflated TEs look strange, my family nicknamed them my "bionic boobies" because they look very mechanical. So don't worry because it is not the final product.

    I actually got my first fill today too, it was not painful at all and I was so happy to have a little 'boob' again!

    Hi Druanne,

    I also had bilateral mastectomy with prepectoral tissue expanders. After surgery I had empty expanders for the first two weeks. My surgeon said that if he is at all worried about the skin he does not put any saline in during surgery. He wants no stress and maximum blood flow. In my case very thin skin was the reason. Everything has turned out just fine. Skin did great! For patients who have less concerns for him, he will start them out with a little saline so they wake up with small breast mounds. I hope all goes smoothly for you!

    I am 10 days out from DMX with te's above the pectoral muscles, I am still having a lot of pain mostly where the sides of the Te's meet my armpits also on top in the center where cleavage would be, my te's are really high, anyone else have similar pain?

    Debsmisto, I had (and still have some) pain around the edges of the TEs. This is where the skin is thinnest and of course I still have some feeling towards the edges where they're rubbing under my skin. Since I had my first fill on Wednesday (5 weeks post op) they are so much more comfortable. Don't get me wrong, they still look weird, and the edges are still a little sore, but I think the deflated TEs had extra ripples in shell that was extra irrating under my skin. But I noticed an improvement in how they feel, comfort-wise. All I can say is it will get better, just hang in there🙂

    I agree. Getting the first fill made the expanders a bit more comfortable. I just got my first post-op fill yesterday. I still have pain when laying down, but in general, my pain level is much better than before.

    I had tissue expanders and hated every minute of them. They looked so fake and hard and touching them just killed (even when my husband pulled sheets at night it would hurt). I had the exchange 3 weeks ago and my breast feel so much better and look way more natural (I had shaped implants because I’m having radiation). Good luck with it all and just remember they will come out

    Druanne how did surgery go? Hope you’re feeling alright.

    I had a left mastectomy with expanders 6 days ago and they are so uncomfortable. PS filled it with 500cc during surgery so I wasn’t flat when I woke up. But I went for my first post op appt and they had to take 100cc out since they thought my skin was not getting enough blood flow. Since they’ve taken it out I’m having the worst pain in my underarm and the top part (so high by the way it’s almost touching my clavicle, not sure what the point of it is) so sore, I can’t touch it. The pain in my Axillary is new and unbearable. I walk around with a pillow under my arm or my arm elevated. Is this normal? They didn’t cut my Axilla during sentinel node resection instead they got the nodes through my breast incision. Any suggestion of how to relieve the pain/discomfort?

    kiso, hopefully taking a little of the volume out will help ensure blood flow to the area. I also had the soreness you describe under the armpit. I couldn't stand even the feeling of clothing. I know what u mean by the pillow..keeping the arm from touching my side helped. Do you still have drains? Once they were gone it was better. Warm showers..massaging the armpit.. with immediate gentle stretching ( once cleared to do so) helped. I had drains for almost 3 weeks so my range of motion was a bit behind. A few PT visits made a big difference. I also was not over filled at surgery so I'm not sure how much or if what you are experiencing is related to that. If it is truly unbearable it's worth a call to the office. Take care

    DebAL thank you for your reply. I do have my drains. One of them is barely draining but the other is a lot. My PS wanted to keep them both in because he was scared my incision may be infected. I’m hoping the drainage will decrease and they’ll take them out next week. At least I’ll be able to go out. It’s kind of weird to go outside with drains hanging on me. My doctor cleared me to do everything with my arm except lifting but this was the resident so I’m going to ask the fellow when I see him next week because I don’t want to overdo and cause more problems. I guess I just need to be patient

    Ther is never enough extra as far as Plastics are concerned , plus they need extra skin for

    Working room to insert real implants. I had same surgery as you Nipple Skin sparing bilateral mastectomy,

    I did not have radiation. My surgery was July 28 2018 , I'm done with the fillups thank God, I'm waiting on my

    Silicone implant 0ct 20th

    Well I’m new to this board but could sure use some advice.

    I had a bi-lateral mastectomy in late February and just saw my PS this morning regarding a reconstruction. After my surgery I had a wound that would not heal. After 5 weeks of drains, 4 more trying to heal I finally had to have a wound vac for 5 more weeks.

    I am now fully healed and thinking I would really like to have the reconstruction surgery. My PS is a little worried about the breast where I had the wound because he said the skin might just be too thin. He did say we could try the TE in both and if for some reason the skin wasn’t cooperating on my right side, he would then do a flap on that breast.

    Has anyone had anything like this in their experience? I am not afraid of the surgery, recovery or the pain. I just don’t want to go through it and have complications like I had when I had the initial mastectomy.

    I did not need chemo or radiation......just on hormone blockers.

    kiso, lol I get it. I walked 3 miles daily with 4 drains swinging. For some reason I did not get as many honks as I used to!

    Jane, that sounds like an appropriate plan. I had no infection or issues but had a discussion with PS about a plan B if there was a complication with the implants. My PS stayed away from lower belly area for fat grafting in case he would need to go there at a later time if there was an issue with implants needing removed. It sounds like he is giving you his Plan B.

    I completely understand your concern due to your previous infection. You have been through a lot. Keep us posted once you have a plan in place. Best wishes to you

    When I had my Tissues expanders in the talk was all about streching out the skin. The skin did ok, it has zeeeero sensation after the surgeries, it's the streching of the muscle that was a problem (my implant is under muscle). Nobody at my PS's office has said anything about streching out the muscle. I don't think I will ever be comfortable with the implant. I don't know how healthy women do breast enlargement, yuck.

    jane, I could see why PS is worried. Implants have high infection rates.

    Thanks everyone for your input. Gives me a lot to think about.

    Where do the incisions usually go for the TE? Some of you said they were very high up. Do you use prosthetics over the TE and if so does that hurt even more?

    I already am having a lot of trouble with all of my clothes.....which is part of the reason I want the reconstruction. I'm also sick of people saying “when are you having the reconstruction surgery?" When will they leave me alone if I choose NOT to have it?

    We live on a lake so summer clothes are SO part of my life. I bought swim suits and some new tops, but I always feel the forms are showing.

    I'm in my mid 60's and my husband is totally leaving the decision up to me. It's just a hard one which I'm sure most of you can agree with.

    Hi!

    Well with all my worry about tissue expanders I woke up from surgery with my final implants already in!!! I am sorry you are having so much pain Kiso!! :( My PS said that during surgery he checked my blood flow and skin envelope and decided they were healthy enough to go direct to implant. I have 790CC Silicone Mentors. I got 2 of the 4 drains removed yesterday and hope to get the final drains out Monday. I am achey and my skin is painful where it is being stretched......worst areas are right on top and directly in between my breasts near my xiphoid process. It really BURNS THERE. I also have a lymph node swollen under my right arm. My PS thought it may be a fluid filled area and poked it with a needle yesterday to aspirate it saying.....this area is probably still pretty numb......Um NOPE.....it wasn't! LOL! AND it wasn't a fluid filled thing, it is a swollen lymph node. I hope it goes down!! I see my BS the 14th.

    For infection my PS has me taking an antibiotic 4 times a day! I was concerned about constipation so I asked the pharmacist what I can do to help and she suggested Florajen Probiotic and WOW.....it has been great so far!!!

    I am 7 days out and overall feel pretty good. Achey and overstuffed a little. These are very large implants. My PS said that the skin was there so he filled it. They do look a little smaller than my originals which is what I was hoping for. I do feel like I look like Barbie on top and hope they settle a little. :) My nipples look a little dark, especially on the radiated side, but he said it is to be expected and looks normal so far.

    What a journey this is!!! Hope you are all doing well today. <3 I followed along on some errands with my fiance to get out of the house and I started feeling twinges of random pains here and there so we came home. I think some nerve endings may be waking up!!!

    I have pockets for my drains that velcro to my bra. I also have a leather belt with two sunflower pouches on it that I stick the drains in once in a while. I also had a cloth belt that I pinned them to for a day not caring if people saw my drains. (I too like to take long walks.) AND before my surgery I bought a "Brobe" it has 4 inside pockets for hiding drains, comes with a halter bra and is SO SOFT!!!!!!!!!!! I will continue to wear it even after I am healed!

    Jane2310 - I don't know if this will help you or not. How important is symmetry to you? During my process for determining reconstruction path, the plastic surgeons I met made one thing very clear - implant will never match natural or a flap reconstructed breast. I mention this only because it reads as if your PS would do flap on your left breast if the TE/implant failed and leave the implant in the right breast. If symmetry is important to you, it's something to discuss with your PS.

    Having decided on unilateral, I knew DIEP was my reconstruction plan from the beginning. My TE was in for 15 months, under the muscle, and I felt it every single day. I'm so happy to have my pec back down on my chest where it belongs and my flap breast on top. It was a big surgery but honestly, I was in less pain and back to work faster than after my mastectomy.

    Druann that’s so great to wake up with implants!! I hope all goes well and no more surgeries for you. Can you tell me the brand of the bra you bought. I tried some before surgery but couldn’t find any that fit right and the one the hospital gave me is very uncomfortable. And it doesn’t help with my pain. I tried exercising it but made things worse.

    Hi! Kiso20!

    I was fitted for a bra assuming I was going to have tissue expanders and then ended up having implants so I have two different bras. Here are pictures of the tags. I am wearing the extra large one because I woke up with implants. The medium fits nice too just not for this healing time. I hope this helps! I LOVE exercising but feel I should relax for another week to heal. Maybe a nice long walk tomorrow!!!!
