Direct to implant reconstruction with skin sparing mastectomy in my opinion is a great way to go if your cancer surgeon and PS indicate that you are a good candidate for this type of reconstruction.

There are 2 reasons direct to implant reconstruction appealed to me:

  1. I did not want to have my muscle cut or involved in any way.
  2. I wanted to have breasts when I woke up from the surgery.

Note that while direct to implant eliminates the need for tissue expanders it does not mean pre-pectoral breast reconstruction. Ask your doctor what his technique is for placing the implant and if he cuts any portion of your chest muscle. Also, the mastectomy pocket left by the cancer surgeon is very large causing the PS to have little to no control over the size of implant required to fill the pocket.

I fell victim to this.

I have since had a revision with a PS who is an expert in direct to implant reconstruction. He has invented the Zenn Delay procedure giving the mastectomy site 2 weeks to heal with the final implant placed pre-pectoral. The healing time allows the PS to control the size of the pocket. To be able to achieve excellent results is well worth the two week wait! I wish I knew about this before my first surgery and I hope this helps others who are in the process of making decisions regarding their own reconstruction.

Google Zenn Delay for more information.
