Triple Negative Chemo suggestion
Hi all,
My sister was recently diagnosed with stage 1 triple negative breast cancer (1.8 cm mass). Her doctor recommends regular AT (A and T at the same time) every three weeks (in contrast to sequential AC-T or a dose-dense regimen). I'm a bit worried that this is a very light regimen given that she has grade 3 TN which can be aggressive, and is multicentric to boot (the other mass is a 0.8mm hormone positive papillory carcinoma), and about 3 years ago she had multifocal DCIS in her other breast and had to have that breast removed. She doesn't have any lymph node involvement or lymphovascular invasion. She had a mastectomy about 3 weeks ago. I was wondering if anyone is aware of any comparative studies. Her doctor actually wants to use epirubicin which I hear is equivalent to Adriamycin. He also wants her to stay on Tamoxifen, which I find a bit odd.
Any help is appreciated.

Hi there superfilbert. I'm very sorry your sister is in this position again. I can only imagine the devastation that came with another dx like this. I was diagnosed with triple neg last month. I started dose dense AC on 5/17. What I know from my oncologist is that he went with dose dense regimen due to my young age (39) and seemingly good health. So, he figured I could tolerate a dose dense regimen. Which so far, thank God, has been manageable. He did say he prefers the dose dense regiment due to the aggressive nature of TN and side of my tumor (2.8cm) as long as I can tolerate it. I did have lymphovascular invasion present. Clear nodes, thank the Lord. Does your sis have any other pre-existing health conditions that might be taken into consideration?
I will follow my 4 dose-dense AC treatments with 12 weekly taxol treatments. I'm not too familiar with the A+T regiment. As for continuing on Tamoxifen....It sounds like your sis has had 2 different types of breast cancers, one that is hormone receptor positive. I think her doctor is continuing her Tamoxifen in order to continue efforts to prevent reoccurence of that specific type of cancer.
This is such a difficult situation to be in for the patient, family and those close to your sis. Things seem to move so quickly with information overload while trying to make the best decisions possible. I've seen many people post about second and even third opinions. I totally support that. I did not go that route. I wish I would have with the surgeon that performed my lumpectomy, but I was so eager to get it out...I just moved forward with the first doctor I met with. My onco I fully trust his treatment plan. Although I may seek another opinion at some point only to discuss long term plans. He seemed in the middle about whether or not I should have a prophylactic Mastectomy.
If there is any doubt then ask, ask , ask questions. I asked my onco many questions and he was very educated when answering them. Sending you all hugs and prayers. I hope confidence and clarity comes your way as your sister moves through this process.

I’d get a second opinion. It seems the standard of care on here is ACT. Also, I also had TN and an ER+ tumor and I’m on Tamoxifen. I’d ask this onc questions, but then I’d seek answers elsewhere if I had any doubts.

Hi all,
Here i would like discuss my mom case, she is detected with breast cancer (3cm) mets to lungs(1cm) & lymph nodes[there were small nodules initially]. Our Onco started with 3 cycles of Chemo [ Alrubicin 160mg, Endoxan 900mg, 5Fu 900mg], after this they did PET scan to check the tumor size, unfortunately it has grown in breast as well as in lungs. Then they decided to give paclitaxel-160mg for 12 weeks (weekly instead of 21 days gap), my mom's platelets count came down after taking taxel chemo for 2 consecutive weeks. Than our Med Onco suggested to go for 21 days gap with Invotaxel-160mg chemo 2 cycles. Again we did CT scan to check the tumor size, we were devastated by seeing the report, tumor is still in progress. Some necrosis were noted in breast & lymph, but more has grown in lungs.
Now we have done with lung biopsy to check ER/PR of lung tumor, waiting for this report.
Request you all to check for chemo medicine my mom has taken so far. Are we in right direction or not? Any suggestions on this would be very helpful for us.
