New Breast Cancer Staging in the US...?
Hi, All. I just had what felt like an 'exit interview' with my nurse navigator after finishing all my treatment. She went over everything that's happened over the past 9 months, including diagnosis and treatment. The surprising thing to me was that I moved from a Stage IIIC (IIIA post surgery) to a IIB. Apparently, there are new staging parameters out there that take into account 'prognostic indicators' - in this case, hormone receptor status. Not that I'm complaining - I'll take it! But, I am definitely cautiously optimistic. Anyone else hear about this? Thoughts? I've pasted the only thing I could find about this from the internet below...

stage 2b sounds much better than 3c. I guess they are trying to make staging more in line with outlook. If you look at the 5 year survival rates they really need to be updated for all stages. People are managing or remaining NED more that ever.

yes, I read something about this a few weeks ago.....would also like to know more about it. Can't remember where I read it or exactly what it said...chemo brain and all!😉 2b sounds much better than 3c....

The decision to delay implementation of the American Joint Committee on Cancer (AJCC) Cancer Staging Manual, Eighth Edition to January 1, 2018 has provided the AJCC with an opportunity to take a careful look at the way it has traditionally communicated cancer staging. Since the manual was published last fall, the AJCC has worked with the surveillance community, the pathology community, and clinical decision support software developers to take a more critical look at the content and make improvements and clarifications that will help everyone who uses this information including the registrar, clinician, and the software developer.
As part of this effort, the AJCC decided to validate and update the Eighth Edition breast cancer staging system using an additional year's worth of data from the National Cancer Database (NCDB). The AJCC Breast Expert Panel has recommended providing two breast cancer Prognostic Stage tables based on further analysis of the NCDB data.
The Clinical Prognostic Stage Group will be used to assign stage for all patients based on history, physical examination, imaging studies, and relevant biopsies. The Pathological Prognostic Stage Group will be used to assign stage for patients who have surgical resection as the initial treatment of their cancer before any systemic or radiation therapy. The Breast Expert Panel also recommended clinical, pathological, and post-therapy data elements that cancer registries should record.
As science continues to evolve, the AJCC is committed to validating and incorporating important updates and communicating them transparently. We understand the burden that these changes place on those who purchased the first printing of the manual. To this end, the entire breast cancer chapter of the manual will be publically available on November 10th through the AJCC website, and replacement pages for all updates and corrections made to the entire manual will be available in December 2017. Future printings of the Staging Manual will include the updated breast chapter as well as other minor updates and corrections issued to date.

well I went from 2A to 1A according the new staging, asked my onc about that and he said as you have one node positive you always gonna remain as stage 2, but its not what I read in AJCC, just confused 🤷🏻♀️

Nas, I'm not sure but I believe that once staged under one system, that staging doesn't change. It will, however, be different for those staged 1/1/18 and beyond. HOWEVER - I can't find clarification on that point. It would be helpful if one of the medical experts on the site could address that question.

My staging goes up under the new edition & I suspect my MO considered that in developing a treatment plan but my official staging hasn't changed. -
