New dx after surgical pathology
Hi everyone,
Just wondering how many have received a different dx after surgery pathology findings?
Best wishes to everyone!
Thanks!

In my case, the micro-invasion (which changes the diagnosis slightly) was found on the stereotactic biopsy, but my understanding is that people are “upgraded” (meaning some invasive cancer, usually just a tiny amount is found in the surgical sample that wasn’t identified prior) about 20% of the time.

Hi Jen -
My surgery pathology was different.
On my left breast the DCIS was still DCIS. However on my right, the lobular neoplasia (was biopsied 3cm ) was actually IDC microinvasion, but changed my staging to Stage1. The treatment plan did not change, but we also did not do a sentinel node excision either like we did on left side....so quite honestly that has been really difficult to accept, absorb, and feel confident about my future.

My pathology was different too. Upgraded from biopsy finding of ADH, to lumpectomy finding of DCIS grade 3.

thanks for all your replies. I am finding this to be happening a lot. Makes me nervous.
Best wishes to all of you!

I believe it's fairly common. My dx was slightly downgraded from high to intermediate grade BUT the surgeon also found ADH along with the DCIS, which was removed as well.

I have DCIS papillary carcinoma, Stage 0, and I thought that was the final word, but when I met with the RO last week, she said that the surgical pathology came back as high grade so she sent the tumor off for Oncotype. Two more weeks of waiting. I've decided that a lot of battling cancer is spent waiting.

Well, my pathology came back. I have IDC now. They didn't get clear margins either. So more surgery for clear margins and limphnode removal or I amy just get a mastectomy with limphnode removal also. Haven't decided.

ReadyAbout, please consider obtaining a 2nd opinion pathology review from a dedicated breast cancer pathologist. Also ask for the Molecular Subtype and the Subtype of Papillary Carcinoma, which will give you more detailed indications.. Some variations of Papillary Carcinoma may have conventional IDC cells in the mix, while others may be "Encapsulated" or pure papillary, being confined to the duct as traditional DCIS. Did you receive a hard copy of your original pathology report? If not, you're entitled to your own copy(s).
Ladies, you're not alone with changing diagnosis, grey areas and differing opinions. I realize the waiting is extremely difficult, so I send you all hugs.
