Hi!

After having my complex cyst for 3 years, the ultrasound yesterday showed it has grown. The Radiologist wants a biopsy. She stressed she was not overly suspicious and felt it was benign but wants to drain it because it has changed since last year. My breast hurts and burns. She said it could be infected or it could have blood in it or fluid. "Overly suspicious" but she is somewhat? I'm a wreck but I'm holding onto her words so tight that she feels it's benign. Would she have said that if she had any doubt? Why has it grown?? So first they do a needle biopsy and if if it doesn't collapse they aspirate, at that point should I worry? Has this happened to anyone else? I'm 60 and past menopause so I'm really scared!

Thanks!!

Maggs, the associated malignancy rate with these is very, very low. Like less than 1%, so when the doctor stressed that she isn't very suspicious, it is because she isn't. Try not to get too worried over something that will most likely turn out to be nothing. Puncturing it or an aspiration would likely make you more comfortable and make the burning and pain stop.

Thank you so much! I obtained a copy of my Radiology report and she listed it as: BI-RADS Category 4 Suspiucious, You can imagine the tears I've shed after googling this! Called my PCP and he gave me Ativan to get through the weekend and advised me to get off the internet but I can't stop reading! The percentage goes from 20-40! I just want it over. The Radiologist didn't give it an A, B or C thank goodness for that! Is it Tuesday yet so I can breathe? THANK YOU SO MUCH FOR YOUR REPLY🙏🏻😀😀😀

the chances that this is benign is 60-80%. Pretty decent.

maggs, I just responded to your concern about the BIRADs 4 in another thread where you commented. Here's what I wrote:

A BIRADs Catgeory 4 means that although the imaging does not have the appearance of cancer, what is seen on the imaging is suspicious enough to warrant a biopsy. Anything that a Radiologist deems to have a 2% or greater risk of cancer is rated as a BIRADs 4, so that a biopsy can be done.

Imaging that the Radiologist assesses to have less than a 2% risk of cancer is rated a BIRADs 3, with the recommendation for short-team follow-up rather than a biopsy.

So if the Radiologist thought that a biopsy would be a good idea just for peace of mind and to confirm that your mass is benign, in order to be the biopsy approved, she had to give it a BIRADs 4 rating. It's a simple as that.

Specific to your comment here, 20%-40% is the average malignancy rate for all BIRADs 4 biopsies; it is not what you were told about your specific situation. You were told by the Radiologist that she thinks this is benign. So for all you know, your Radiologist might have assessed your risk to be 3% (or 5% or 7% or...), which still warrants a biopsy and still warrants a BIRADS 4 rating.

Your PCP is right. Stay off the internet. You are scaring yourself with general averages that might be completely irrelevant to your specific situation.

And if you absolutely have to read something, read this, particularly the section on BIRADs 4 and the description of 4a. http://www.radiologyassistant.nl/en/p53b4082c92130/bi-rads-for-mammography-and-ultrasound-2013.html
