Can DCIS reoccur in NSM?

I had a grade 3 left DCIS DL in 2013 and had er-/pr- pathology. Was very fast growing and bled out of my nipple and breast enlarged. Had large area of DCIS behind nipple and outward at 3:00. My surgeon INSISTED on me doing a NSM on my non cancer side. No one checks for reoccurrence on that side, well they don't check me on either side, just annual exam. .. I want it off, it's not worth it, has anyone heard of reoccurrence of DCIS/IDC on NSM side? I did have atypical ductal hyperplasia cells on my non cancer side pathology.

    Mimi, DCIS, or even invasive cancer, can develop after a NSM. But DCIS and/or invasive cancer can develop after any MX, whether nipple sparing or not. This is because no matter how expert the surgeon, a small amount of breast tissue will always remain after a mastectomy. And where there is breast tissue, breast cancer can develop.

    On average, the risk of recurrence after a MX, whether nipple sparing or not, is approx. 1% - 2%. A recurrence is the redevelopment of the original cancer in the original location.

    Separate from this, there is also approx. a 1% - 2% risk that a new primary breast cancer might develop. This would be a new breast cancer that is separate from and unrelated to the first diagnosis. A new primary can occur in either breast. Since you are asking about your non-cancer breast, it sounds as though this is the risk that you are asking about.

    So yes, this can happen, but the risk is low. Removing your nipple (which I think is what you are asking about) likely wouldn't change the risk by much, if anything at all.

    Note that the 1% - 2% risk figures I've mentioned are averages for all women who've had a MX or BMX. Individual risk for any one person could be higher due to factors such as being BRCA positive or having very narrow surgical margins, etc..
