Possible hysterectomy—opinions

Hi everyone. I'd like your opinions. My GYN (she also talked to my MO who is also on board)thinks I should have a hysterectomy with ovaries and tubes removed but I can't help but wonder if it necessary. I am 40 and done with babies. My BC was ER+ and PR+ and my periods stopped during chemo but they've come back full force. I've had a biopsy and D&C in the last year because of the bleeding. FSH numbers are elevated and estrogen is normal so she thinks I'm perimenopausal. Any thoughts? Particularly interested in anyone who’s had the same surgery and what their side effects were.I'm scared of yet another surgery but more scared of cancer coming back. Anyone premenopausal ever have a hysterectomy?

    hi Carrie

I have been wading through the hysterectomy information myself because of fibroids mostly. In my particular case my gyn was strongly suggesting uterus and Fallopian tubes but leave ovaries. Saw a gynecologist oncologist and he recommended the da Vinci which he has done over 1700 surgeries of and said I was grey area to take or leave ovaries being perimenupausal. And history of BC. Then I saw my MO. He said in my case I should leave the ovaries. I am low risk for ovarian cancer and Soft/text trial saw no benefit in removing ovaries for someone with my stats. The cardiovascular, dementia, bone problem risks were not worth it for me. So I feel I have talked to all the “experts” I could and will make the appointment for surgery next week but i am dreadin

Thanks for replying! I think I want to talk to my MO too and ask some more specific questions.

    Hi Carrie37:

I have had hysterectomy/oophorectomy after chemo stopped my periods. I was 41 years old at the time. When chemo ended and I was on tamoxifen, my periods came back with a vengence. I had several D &C's actually because they were so heavy. My uterus was like a large soggy bag I was told, so hysterectomy was recommended. And, MO was going to shut down my ovaries chemically anyways to give me aromatase inhibitors instead of tamoxifen, so I had my ovaries out too.

Like you, I was terrified of what this would do to me.

I am now 54 and have no issues after the hysterectomy/oophorectomy. I was delighted with my periods ending, which surprised me because I thought periods meant "youth" and "health". But no periods is really freedom, I must say. My PMS disappeared too! I also thought it would age me faster too. But just the other day, a GP who I had never seen before that I went to for something, thought I was 40 years old and premenopausal. So, thats not bad right? I look 14 years younger than I am!

The surgery was open abdomen for me, so I have a scar along my bikini line which is barely visible. I was in hospital two nights after the surgery on morphine. After that, I took tylenol. I was very tired though. It took me a good 6 weeks to recover.

If you want to PM me, please do.

Speaking to the surgery itself and side effects, I had none. It was done laparoscopically. The surgery itself was about 45 min. I went in the morning and was home by 6:30 pm. The first night and next daywere painful. Felt like bad cramps.

I just had all the baby making parts removed including cervix Nov 2. Lap-vag procedure. Very little to no pain afterward. I’ve had a little increase in amount of hair in the drain when I shampoo, memory issues and fatigue. This may just be my body adjusting to the new normal (without estrogen). Hopefully it’s temporary (had the same symptoms on tamoxifen too). Hands down it was the easiest surgery I’ve ever had (and I’ve had a lot of them).

Can those of you chiming in state why your doctor (or you, in making the decision) chose hysterectomy and/or oophorectomy? I'm facing ovarian cyst issues, which I've never had before and have had previous ultrasounds, so I feel it may be Tamox. related.

My gyno suggested hyst./ooph in a passing comment (as we scheduled another ultrasound appt). However, she does not deal with a lot of BC, and I'm going to discuss with a gyn/ocn, too.

But for now, I'm just researching. I may have this issue now to deal with. Fun times in BCLand.

    Thanks!!!

In my case, the cysts looked suspicious for ovarian malignancy on CT. And, since I am BRCA2+, all 3 gyn oncologists I consulted with, recommended the ooph. They all said that if I was not BRCA+, they would have observed the cysts for a few weeks.Luckily, everything turned out benign. I grew 8 cm and 5 cm complex cysts just within 6 months from the last TVUS, I attribute this to tamoxifen.

Hi I am sitting here recovering from my hyster (cervix, ovaries, uterus, tubes ). I had a robotic hyster, 4 small incisions, and one internal in my vagina. I had this hyster because of cervical cancer in situ (Stage 0 - and precancer in the gyno cancer world). I was perimenopausal, high FSH, and the big decision was not whether or not to complete the surgery BUT was I going to keep my ovaries....my BC was 100% ER and PR+, my sis had uterine cancer, my father had 3 cancers, and I tested negative for the Cancer 34 Panel from Ambry Genetics....so I had medical opinions that varied, but in the end the gyn-onc and my BS plus the BO at City of Hope helped me make what they said "was the safest decision and would lower my risks but how much would not be able to be calculated." I am 47yo. My side effects --> can't remember sh*t, which my gyn-onc says will pass....BUT everything else --OOOOH my gosh I feel so much better!!!! Seriously this was so unexpected....I am very happy at this point, and hope it continues.....I am 5wks out.

My path results --> another lesion in my cervix, fibroids in my uterus (small ones), cysts on my right ovary (small ones...did not know they were there) and hyperplasia in my uterus....afterwards my gyn-onc said she was so pleased I pushed her to get this scheduled in this calendar year.....I will see her again next year....she did counsel me about vaginal dryness and Oh...the hot flashes..started about two weeks after hyster and came about every 45min on Thanksgiving and the day after and now I have one maybe a day....

I tested positive for the BRIP-1 gene mutation which means increased risk for ovarian cancer. Then a couple weeks later I develop blood clots in my left calf and both lungs-attributed to tamoxifen. So couldn’t take tamoxifen anymore and options were to shut down the ovaries for the next 5 years to take an AI, have oophorectomy and start AI it do nothing. Once oophorectomy was decided, OB/GYN said uterus had to go too as I was on tamoxifen which increases risk for uterine cancer.

Thanks, everyone, for chiming in. It's helpful to gain some insight into an area I was not really expecting to have to gain insight into. F-ing BC! (Exchange surgery just over in Sept, so thought I had a moment to breath, and now ovaries are not playing nice.)

I have osteoporosis, too, to deal with. (Not unexpected as I'm very petite but I'm kinda young for it...) So have to throw that in the mix, too. Just getting to endo appts now. So I'm left somewhat with limited options. I have another TVUS next week.

Happy holidays to everyone! You're my rock stars!!

thanks everyone for chiming in on this topic. My surgery is scheduled for January 25th. I’m nervous about side effects but more nervous about possibly having BC return.
