Can anyone recommend a nipple sparing mastectomy surgeon in NY? Im a 34DD and do not want to have a reducution first; I also dont want to have expanders.

I haven't consulted with anyone yet. But from what Im reading I think Dr Salzberg would require me to have a reduction first and then he would do direct to implant.

I also think Dr. Karp only uses tissue expanders? (so that would be 2 surgeries).

Is this info correct? Anyone know of anyone who can do it without reduction and direct to implant?

    My PS felt direct to implant was best option for me. I'm thin and tight skin and he said to use expanders would not be a viable option. DIEP was similar in that I did not have enough lower fat for it and I did not want to use thighs or butt and have scars there too. \

    If I have this right, which ever boob is removed(or both), tissue expanders are put in and then filled until desired cup/cc size then swapped out for the implant in the 2nd surgery. I'd say you probably could not get it to match closely without a reduction unless both boobs are being removed. I could be wrong. What about DIEP? You could try posting in the reconstruction forum if you havent already for any questions on reduction etc.

    Hi Randi...I had direct to implant reconstruction with Dr. Christine Rohde at Columbia University Medical Center. IMO she is a brilliant surgeon. Good luck and keep us posted.
