Needing opinions here.  I have been around this forum for several years, always coming back to take advantage of the wealth of knowledge everyone has.  I had a routine MRI on breast 8/21/17 that ended up Birads 0.  I have had 6 biopsies, and 4 biopsy aspirations (radiologist wanted aspirations sent to pathology).  All have been benign.  This MRI is by far the most concerning.  Here are results:

Oval enhancing mass demonstrates irregular borders, mild high T2 signal, with intrinsic rapid initial enhancement and mixed delayed plateau and washout kinetics (type II/III enhancement curves).  Birads Category 0, additional targeted second look ultrasound evaluation for 9mm oval enhancing mass needed.

What are the chances of this just being another fibroadenoma or cyst?  Side note:  The report also stated Extreme dense fibroglandular tissue with marked background glandular enhancement, so busy breasts, no surprise.  No first degree BC in family just 2nd and 3rd, but my twin sister did have a mastectomy for ADH, ALH, PASH, and FEA all bilateral.

Do you think I will have another biopsy?  Ultrasound is tomorrow morning.

BIRADS 0 indicates no risk of cáncer, I think you may just have an ultrasound, its possible you may have something like PASH

No, that's incorrect. BIRADS 0 means "incomplete" i.e. additional work up is needed. BIRADS 1 means negative for findings, BIRADS 2 means b9 findings (both are no risk of cancer)

I have been through enough to know that Birads 0 means needs further imaging.  Since 09/2011   I have had 10 Mammograms, 11 Ultrasounds, 4 MRI's and 10 biopsies.  I had an Ultrasound yesterday, it took almost 2 hours.  The "targeted look" required scanning the entire breast since so many areas matched the MRI description.  In the end I thought for sure I was going to be told to come back for another MRI in 6 months.

Today I received a call from my Doctor stating there is at least 1 area that needs biopsied.  Not surprised.  My original question referred to the not so great characteristics that describe my "mass" on the MRI.  I know irregular borders can be bad.  High T2 signal is not always bad.  Rapid initial enhacement and mixed delayed plateau and washout kinetics (type II/III enhancement curves) now THAT is bad.  I

I have always hung onto the 80/20% rule and just assumed I would be in the 80%.  So far so good.  Until now.  Can anyone guess what my new 80/20 numbers will be?  Looking for a ray of sunshine to hold onto here.

    Thanks.

Sorry VanillaPudding, I don't know what your specific numbers would be, but I do know that 70-80% of biopsies come back b9. I'm hoping you're in the majority!!

Well, after ultrasound I have been upgraded to Birads 5.  Not good.  My biopsy is scheduled on the 21st but they are trying to get me in faster.  Poop.  On a happier note, if I can make it through this biopsy and by small chance it is not Cancer, my insurance will approve a prophylactic Mastectomy.  So, off to research I go.
