    Hi Jnnmn7,

I had direct implants under the muscle with a nipple sparing mastectomy. I have no regrets. As too look and feel. I was small, very small busted to begin with, not much volume. My PS said my skin was tight so using expanders was not an option. So he put in small implant. It gave me volume so that is a plus. As for feel, my skin is still sensitive but if I touch/squeeze them it's not as if they are hard. They just feel firm in a good way but also the muscle is over them. They look natural. I knew I would get a small implant but I wasn't sure how it would like either until it was done. I'm about 2weeks out now.

I'm five years out from my exchange. In general I'm happy with the results. I have implants that are a similar size compared to the tissue that was removed. I have 3-D tattoos from Vinnie Myers. My pecs took a long time to settle down. Now I don't notice the implant is under the muscle unless I purposely think of it. The scar along the upper pole has tightened and may have some encapsulation. My PS said she could fix it, but I'm not interested. I never wear a bra. That is awesome. They look and feel natural to me. They are cold in the winter which tends to chill me to the bone. I have feeling on the skin of the breast mounds, except or about a one inch area along the scar.

I am very happy with my results. The implants look great - more like implants than real breasts, but they do look nice! They definitely don't feel natural, though. They feel like implants to the touch. Not a big deal to me. One thing that I was sad about was that having the implants under the muscle has limited my weightlifting. I've been lifting weights my entire adult life and my favorite exercises were pushups and bench presses. My PS said I could continue to do them but I noticed a very distinct change in the position of the implants after doing those types of exercises and they did not return to their original position. I have since modified my workouts to do more isolated arm exercises instead. It's a minor thing, but it did matter enough to me to mention it in case you are a weightlifter.

    Summer:

I love weights but do very -- I think compared to your routine -- simple classes and light weights. I also used to do push-ups and plank, etc., but have limited somewhat for the time being. I'm still in TEs with Exchange surgery in 2 weeks.

Did you ditch push-ups altogether? What else is not a good idea?

I'm doing tricep/bicep targeted with light weights and it's helped arms since losing out on quitting push-ups. After Exchange, gotta be careful for a bit, too. Sigh.

    ;)

I did ditch the pushups (and planks and bench presses). It's mainly because those exercises are VERY contracting to the pec muscles. I still do other exercises that work the pecs but not as hard, like lat pulldowns. Some women have no trouble at all with pec exercises, though, so it may just be my issue. I didn't specifically avoid any other exercises, but I did start back up very slowly after my surgery to let my lymph system heal and avoid lymphedema.

I don't have any issues with using my pecs. I'm lifting heavy stuff around the farm all the time. The pecs distort but return to their position. I have 421 moderate projection so they are wide and flattish which is great by me. I think I had such a hard time with the TEs because my pecs are strong. I only had one fill of 150cc beyond the 200 cc at the mx. It was excruciating. I threatened to take them out a thousand times before the exchange.
