Scar Placement for SGAP, Different Results?

Why are the SGAP scars in different placements on the buttocks in various women? Does the scar placement make the butt more or less difficult to reconstruct in stage 2?

    I don't know for sure, but within the same surgeon's practice I would think it would have to do with how much tissue is available and/or how much breast they're needing to reconstruct. The difference could also be a between surgeon difference. I know that different surgeons place the DIEP flap abdominal incisions in different places-some are placed at the belly button and others are placed lower. The lower ones are obviously easier to conceal. In my opinion, the surgeon should be cognizant of the fact that most women would want to be able to conceal any scars (especially early on before they have a chance to fade) and should plan the procedure accordingly. This is part of why before and after pics of work the surgeon you're considering are so important to see before letting them do the procedure. If you look at the pics and don't like what you see, don't use that surgeon. Same goes for deciding between types of recon or selecting none. We deserve and owe itto ourselves to know what our expected outcome will be

    Here is more information on SGAP from our main site that may be of help. And also, there is another type of GAP flap, the IGAP (inferior gluteal artery perforator) flap, which uses tissue from the bottom of your buttocks, near the buttock crease (follow the link to learn more on this type of reconstruction).

    My scar is in the middle of my butt. The doctor knew my concerns about flattening my buttocks. He told me that was where the perforators were and he would fix me in stage 2. He places scars in various places in different people. I trusted he knew what he was doing. Well, stage 2 has come and gone. My buttocks is not only flat, but is also distorted. Has anyone else experienced this? Do I have to find another doctor, one that specializes in buttocks repair? How do I find such a doctor? Has anyone been in my postion, not happy with sgap donor site revision? What did you do

    Hi AD197 - I am in the process of researching reconstruction options and have consulted with a few surgeons about this. I too am curious about where you surgery was performed. One plastic surgeon told me that the placement of donor site incisions depends on the location of the specific arteries available at the donor site. Apparently the entire success of the surgery is dependent on the viability of the arteries in the donor flap. And apparently the SGAP arteries tend to be smaller than the DIEP flap arteries, and that makes the surgery more challenging. Two very well regarded surgeons both told me they would recommend I travel out of state (to NOLA) for SGAP because it is such a challenging surgery. I hope this info helps and I hope you will get results you are happy with!

    meesha - I can tell you a lot about the sgap. I was ill advised and NOW I know that the sgap was the worst place to take fat from me. I was told it was because I didn't have enough belly fat. I also didn't have enough butt fat and I look really bad. I regret my decision. I wish I was told the truth. I am searching for women that had my same experience, but got it fixed. I want to know what procedures they did. No one talks about their butt. I don't want anyone else to make an uninformed decision like I did. If the doctor takes fat from the center of your buttocks, you will have a flat and possibly deformed buttock, even after stage 2. Pear shaped women can get away with it, but not slender women. They can still take from the sgap perforators, but design the flap to fit the woman by placing it higher or on the sides. You can also use the lumbar perforators which are higher on the hip/bac, but I think the sgap is easier for most doctors. Read Dr. Greenspun's website that warns about taking away fat from the buttocks, something I was told would be fixed in stage 2 and I would not have a flat butt. I trusted my doctor, even though I thought the markings were too low, he just got done telling me he wasn't going to take away my buttock. I didn't have any experience, but he was the expert with lots of experience and knew what I wanted. If I could do it again, I would go to NOLA and get a hip flap with Dr. Sullivan. I don't think NOLA would be able to fix my butt now the damage is done. Sgap is for pear shaped, hip flap is for slender women. Don't make the same mistake I did.

    Has anyone used a cosmetic surgeon to fix their butt? I don't trust my current surgeon, as a good doctor would not destroy the buttocks to begin with. He has to consult with another doctor before he can talk to me about how he is going to get me back to a normal shape. That is not a good sign. I know I am not the only one that had their buttocks deformed by sgap.

    When is the SGAP scar that goes across the buttocks fully healed so it will not split open if fat is gained in the buttocks?
