Screening MRI after bilateral mastectomy with implants?
How often (if ever) do you have MRI's after a mastectomy with implants? I was hoping to have them annually for peace of mind and I had one last year but my employer changed insurance carriers and my new carrier has denied it.
Does insurance have to pay for a MRI without contrast atleast every 2 years to look for ruptures?

I was told by my PS that every two years either MRI or ultrasound needs to be done to check for rupture. That's not the same as MRI for recurrence. I don't know what the screening is post BMX.

I have read that it's pretty common for insurance to deny MRIs that are meant to check for leaks. My PS stated that he prefers an MRI after 3 years and every 2 years after that, but that insurance coverage varies.

Thanks for your replies...it's scary to think that they feel we shouldn't have any formal screening for a local reoccurrence even if the odds say there is a small chance of that happening.

I agree. I think MRI screening needs to be done, particularly for reconstruction over the muscle, since the chest wall is now hidden behind an implant and therefore even a professional breast exam can't detect it.
Also, it's my understanding that the type of MRI done to check for implant rupture is done without contrast and therefore won't detect recurrence. So, basically, we are left with no screening

I have an MRI every 2 years to check for ruptures. I have the literature from the manufacturer that was given to me by my plastic surgeon. The manufacturer recommends MRI every 2 years, so my insurance pays for it.

Thank You, Rambros for starting this thread. It's exactly what I have been looking for. My implants are more than seven years old and I have never had a follow up MRI. Recently my right armpit has had an almost constant ache. I cannot feel any lumps, there is no redness or skin abnormalities. The breast itself looks the same as it has since a few months after surgery in 2010 Unfortunately my wonderful BS is retired now and my PS has moved out of state. I am not happy with the Breast Dr's in my area. I see my PCP on Monday for my regular check up. I think it's time to get this checked out. I believe he will order anything I need, but BC/BS insurance can be a pain in the butt to approve it.

Anyone know if Medicare covers it? Did not know MRI is recommended for check for ruptures. I'm not 2 years out from implants yet but good to know these things.

Artista - Medicare covered an MRI for me 1) prior to initial BMX, 2) one year after BMX, 3) when an ULS found the recurrence at 2 years, 4) after first 6 rounds of chemo & before ALND surgery The next MRI to check implants is recommended at 5 years & then 10 years. The MO I loved also retired. I chose to fight the new MO for PET/CT instead of MRI after final treatment & again one year later. This new MO is big on regular CT scans with & w/o contrast. I had one every 6 months for the first 2 years. Medicare paid everything with no problem. I've pretty much made my mind I'm done with scans unless I think there is a problem - then I'll push for ULS first and then PET/CT.

I was told that the type of MRI that MIGHT (and might not) be approved to check for implant leaking/rupture is without contrast, and therefore would be useless to pick up recurrence. Is that true?

how about ultrasound? will it detect recurrence in chest wall

Candles - sorry I don't know. My MRIs were all both with & without at the same time. Once you're there and in the machine, a few more pictures w/contrast don't seem like such a stretch.
Kae - I don't know the answer about ULS either. My guess would be that the implants might block the view. Hopefully someone who has more knowledge will check in soon.

I think only a MRI with contrast would detect cancer, without contrast will check for any implant ruptures - but I think if you get one with contrast they always do with and without in the same scan. My MRI has been denied (even after appeal), b/c they say I would have to have a mammogram, ultrasound or biopsy first. I still want one though so I will have to pay out of pocket. I hope others who want a MRI are able to get theirs covered (I had no problems with my prior insurance, BCBS)

My PCP ordered an ultrasound of my right Breast and armpit. We talked about an MRI to check for rupture/leakage. He said that if I really wanted it, he would be happy to order it. But that he doesn't feel it is necessary. He explained that unless I had an obvious breast malformation (I don't) that a small leak would not be harmful. And further surgery to remove and/or exchange the implant would cause more scar tissue and the possibility of having an undesirable outcome would be very real. So I will stay with his advise unless things change.

NavyMom- My ps sounds like yours. He doesn't want to mess with things unless it looks necessary. Seems everytime someone touches me I get infected so I'm with you. No need unless something looks or feels suspicious.

And now I wait. It takes 2 weeks to get an appointment for the U/S. Grrrrr! I hate the wait. Then of course it will take 3 days(if I'm lucky) to get results. I'm not overly worried....just need to make sure things are OK. I had a axillary node that showed up on a post treatment PET scan. It was believed that it was inflammation from the mastectomy. A year later it was gone.
I'll be sure to post again when I get more information. Thanks to all for being here.

Axillary U/S done today. Results already posted in my patient portal. States that there is a 1 cm lymph node that looks benign. BIRADS 2.
Should not be of concern. I think I will request a f/u US sometime in the next 3 or 4 months just to make sure it hasn't changed. That sounds reasonable, yes?

Thanks, MT. My PCP has agreed to my request for a follow up ultrasound. Already scheduled for October. So I'm just gonna forget about it until then.

I had my follow up ultrasound of my right axilla. The lymph node is still present and the same size. Radiology is convinced that it is benign. I'm good with that.
