I recently had a 5cm tumor removed stage 1 via mastectomy. My doctor did not have me go thru chemo because she thought it was stage 0. Lymph nodes were clear. Should I worrry that I did not have chemo? Thamks

Confused - stage 0 would be DCIS. Stage 1 would be IDC and would normally need chemo or rads. You need to clarify this, as well as your hormone status and HER2 status. All those things make a difference.

I am new at this so I may not give all of the facts. Prior to my mastectomy I was told I had in situstage 0 with 5cm tumor grade 3. After surgery I was told the cancer was not contained and approximately 1mm was found outside the tumor. After surgery I was informed I was triple niegative and had Pajets on the nipple. Everything I've read indicates I should have gone through chemo for trip neg but my surgeon said it is not necessary. Did not see the chemo doc until after surgery and was told it's too late for chemo now. I'm confused about the treatment and if others skipped chemo for large tumor clear lymph nodes stage one tripled negative. Sorry if I'm not technical enough like I said confused.

    Hi Confused,

My understanding is that chemo is recommended for all TNBC IDC that is 5 mm or larger. And that chemo is not indicated for DCIS. So since your IDC was only 1 mm, it does not surprise me that they did not recommend chemo.

However, take all of this with a grain of salt--I am not a doctor. If you aren't happy/comfortable with what your doctor told you, a 2nd opinion could make a lot of sense!!!

The one thing that doesn't make sense to me is the part about it being too late for chemo just because you already had surgery. While many of us TNBCers have chemo first (called neoadjuvant chemo), plenty of patients have chemo after surgery (called adjuvant chemo). When did you have your mastectomy? Was it recent? But regardless of the timing, they might just believe that a 1 mm tumor doesn't warrant chemo.

I am sorry but I don't really know much about Pajet's disease.

I hope this is a little helpful. Mostly I think a 2nd opinion is a good idea; I think your case sounds a little complicated what with the DCIS, the IDC, and the Pajet's.

    Good luck!!!

Thanks for your response. I think a 2nd opinion is a good idea. The primary tumor was 5cm and the cancer outside of the duct was 1mm. I'm not sure if that makes any difference in regards to chemo but I think it is worth getting another opinion.
