I have a 7mm focus of dcis, int-high grade and +comedonecrosis er/pr+, diagnosed via stereo core bx. there are some other calcs bilaterally that rad isn't worried about, but I have decided to get them all wire loc'ed and removed via lumpectomy, then will have breast lift bilaterally (currently huge boobs dd+). if the path comes back just dcis, I may stop there and not do xrt or tamoxifen, but im nervous I may get upgraded to invasive and then will need the full monty for treatment. I like to plan ahead, so this unknown makes me nuts!!

any ladies out there go in for dcis and get path that bumped you up to invasive? what did you decide for treatment? or anyone with int-hi grade dcis who deferred xrt?

thanks and happy holidays - may the new year bring us ALL health and happiness!!

Things are really slow here on holidays, but statistically about 20% of cases with DCIS on biopsy end up with invasive cancer on final surgery

I didn't get "upgraded," but it's easy to understand how it could happen. Take a look at this image from the main site:

It becomes clear both why it's important to treat DCIS, and how it can continue to develop into invasive disease.

With any luck, others will be along to discuss how they responded to an "upgrade," but there's no sense worrying about it in advance of your post-surgery pathology report. Good luck!

I didn’t get upgraded after surgery as my micro invasion of IDC was found on the initial biopsy along with the DCIS, but the only difference it made in my treatment was needing a sentinel node biopsy. Radiation and tamoxifen would have been recommended in my case even without the micro-invasion (in part because of my age), and the IDC wasn’t large enough to warrant chemo, which is usually (but not always) the case in those 20% of DCIS cases that are upgraded.
