Pathology availability
I am two days out of surgery and had a unilateral mas and 12 nodes removed. They didn't tell us how many postive which i was surprised. I have to wait a week or at least a few days.
Is there any indication of how many are positive based on how many removed ? I had a sentinel node biopsy to confirm postive so I know it's at least one. Doc said based on MRI it could be only the one. Hown common is jt that MRI picks up positive nodes accurately ?

MRIs are not reliable indicators of nodal involvement. An MRI cannot detect anything smaller than 2mm-3mm in size, so if any nodes have smaller areas of cancer, this would not be detected by an MRI.
Similarly, nodal involvement would generally not be detectable to the naked eye (unless perhaps a node was completely taken over by cancer) so the number of nodes that the surgeon removes is not an indicator of how many nodes may be positive. Lymph nodes are grouped together in levels, and usually a surgeon will choose to remove all Level 1 or possibly all Level 1 and Level 2 nodes. We each have a different number of nodes by level; with 12 nodes removed, I would make an uneducated guess that your surgeon may have removed all your Level 1 nodes.
It takes a detailed assessment under a microscope for a pathologist to detect the presence of cancer cells, and often specials stains are necessary to clarify what's seen. This takes time. This is why full pathology results, both for the nodes and for the breast tissue, are usually not available for at least several days after surgery.

Just wondering if your surgeon said how long it takes for path? Do you wait for the path results before they advise next treatment step? Hope you are doing well after your surgery. I am a few weeks behind you, so appreciate any tips/etc that you encounter on this step of this journey!
Wishing you fast healing. I feel this period of time between DX and surgery is soooo long for me. So hard to rest my mind about all of this.

My husband was called while I was still in surgery and informed the sentinel nodes were positive. The pathology report will take 5 to 10 business days.

I have not heard yet. My post op is Friday! I will know more then.
Tiglio - the worst part was waiting for surgery...that's what everyone said and it is true. the surgery went well..now we go onto the next waiting which is pathology and then starting chemo...im scared of the side effects of chemo but trying to go one step at a time. when id your surgery?

hi scaredashell07---I dont have a set date yet, so its driving me crazy. the waiting and waiting is pure torture for me and my family. I meet w PS Monday and then should have a date finally set next week. I am super nervous about if i will have to do chemo too. Will they tell you the specifics of chemo at your post op? I am glad you are doing well healing from surgery!
Will be interested to see how close our treatment plans are since we have simliar situations it appears.

hope the surgery gets scheduled soon for you. Chemo discussion happens in a week or two. I am actually more worried about the chemo than I was about surgery but one step at a time
