Inconclusive Oncotype Test (HER2- IDC/ HER2+DCIS)

My bioposy result was Her2+ as a result of FISH test. I had a lumpectomy on 9/19/16. Tumor was a mix of invasive (IDC) and non-invasive (DCIS) and about 1 cm. Pathology results showed I'm 95%+ estrogen positive, 95%+ progesterone positive and, after FISH, HER2 NEGATIVE instead of positive. The pathologist thinks original biopsy included some non-invasive cells in the sample. The sample was then sent to get an oncotype score since I was now determined to be Her2-. I waited 3 looong weeks for the result and it came back today as inconclusive because of the mix of Her2- invasive with Her2+ Noninvasive. I had an appointment with a well-regarded oncologist this afternoon and he says I don't need chemo based on other factors (tumor is just under 1 cm, lymph nodes clear, Estrogen and progesterone are so strongly positive and Her2 status for invasive is, to him, clearly negative). Has anyone had this happen? Onco test people say it happens less than .01% of the time. I would love to avoid chemo but don't want to take any risks. Thanks!

I don’t know about it happening the way it did for you on the test, but I do know that DCIS is much more likely to be HER2+ than invasive cancer is. No one knows why and HER2+ doesn’t seem to be as significant for DCIS as it would be for IDC.

What you were told sounds consistent with that, so I would be inclined to trust the oncologist, but if you are really worried get a second opinion.

Hi, I didn't get an oncotype number. The result was just "pathology failure", they told the oncologist they couldn't give me a score because they couldn't get a large enough sample of just IDC cells. However, my oncologist called an oncologist at the lab and after speaking about my case in more detail, they are retesting me and I'm hoping I can get a score in about a week. Radiation is being delayed until they figure this out
