Armpit lump above Sentinel Lymph Node Biopsy

It's been 4 years since I had chemotherapy, bilateral mastectomy & reconstruction. Recently, I have noticed that my lymph nodes under same armpit as cancer breast is strange. Feels like the end of a rope that ends right where my biopsy scar is. Is this normal? Do they actually cut the lymph nodes and perhaps this is just a detached end (under skin) that has scarred? Lymph nodes on other armpit feel same but with no detachment feeling.

This is what the lump has grown into in the last cpl of months. At first it felt like a small bb under the skin. No outward sign meaning no rash or redness (until the other night when I dry shaved like a dork cause I wanted to better a better feel of under the skin). Has anyone else had something like this happen and what was the result? My bmx was Feb 2013 and my last reconstruction surgery was November of 2015. My us is tomorrow but I'm anxious to know.

They could not see anything in the ultrasound. I'm hoping that's good news yet it's obviously there and can be felt so I don't understand. I meet with the surgeon Wednesday
