How soon did you start hormonal therapy?
I have just finished neoadjuvant chemo (yay!), booked in for surgery in three weeks, followed by six weeks radiation. My oncologist has said we will talk endocrine therapy after rads have finished.
I feel nervous about having three months between finishing chemo and starting hormonals, but do have a lot of faith in my medical team.
Has anyone else had is experience who might like to help calm my fears?

I didn't have chemo but I did have radiation. 33 treatments. My oncologist said I wouldn't start Tamoxifen until rads were completed. Apparently SOP so don't worry. You trust your medical team. That's important.

No chemo here, but started AI on New Year’s Eve, a month after I finished rads (didn’t want letrozole’s SE’s to kick in while I was on a Mediterranean cruise). My MO was fine with that.

I had neoadjvant chemo, surgery and now radiation. My MO was gong to wait until after Rads too but I insisted that I start--I'm 100% ER. She said that there was no harm in starting and we could always stop if problems arise. However I did the FSH and another test to ensure I was postmenopausal. I'm on Aromasin--take before bedtime--and have a week of radiation left and doing pretty good.
Talk to your doctor about your concerns. Its also very likely that holding off for another month or so isn't a risk. I'm sure you'll be fine with whatever you both decide.

It's pretty common to start hormonal therapy after radiation, this way they can isolate side effects. Some MOs want to start sooner (between chemo and rads). I started mine 3 weeks post final chemo and 10 days before rad which was fine by me.
-
I started before rads.
-
opt4life - I am 95%+ ER and PR, but I'm also definitely not post menopausal!
I'm pretty much having the works with my treatment, so isolating side effects does make sense.

I started anti-hormonal therapy two or three weeks before rads. My doc gave me the prescription at my last chemo appointment and told me to start any time I want.

The reason you don't start the tamox/AIs until a few weeks after rads end, is because the whole point of chemo & rads is to kill the fast-growing cells. Which is what cancer cells are. (Some normal cells are, too, like hair follicles, which is why we get the SEs.) Anyway, the purpose of the anti-hormonals is to starve the cancer cells - hence they are no longer growing fast - so would not be killed by the chemo or rads.

My MO wants me to start now (post chemo, pre-surgery & rads). I think you could ask your MO about it. It might calm your worries.

I started Arimidex 4 weeks after my last chemo infusion, just before rads. I was also already on zoladex to protect my ovaries throughoutchemo.

@BlueKoala, My cancer and treatment plan are very similar to yours. I knew prior to the start of treatment from a biopsy that the cancer had spread to at least one lymph node. The chemo did dramatically reduce the scope of my cancer but it was not a complete response. The treatment time frame is basically the same as yours. The difference is that I started Arimidex during my radiation therapy - probably around the second week or so. It was actually the RO that suggested I begin the Arimidex. I told my MO that the RO had asked me several times if I had started it yet (and I hadn't). My MO said it was ok to begin Arimidex, so I did.
@Jennie93, So are you saying that all those radiation sessions I had after beginning Arimidex were for nothing? It did nothing to kill any left over cancer cells? I have to say you're freaking me out with this information. Do you have any references to where you learned this information? You have me very worried.

I finished my 5+ months of chemo and then started tamoxifen in April, at the same time I began rads. No one (RO, MO, etc.) seemed to be concerned about WHEN I started tamoxifen, and no one mentioned that the med won't work if I got rads at the same time or that rads efficacy would be compromised by the medication.
@KayaRose,the above explanation about not starting meds till rads were done sounds a bit oversimplified, and I wouldn't worry about the order you began meds and rads (like me, close together).

That is what my RO & MO told me when I asked about timing. It makes sense.....
They also told me not to take any supplements like antioxidants and such, during treatment, because it might protect the cells you are trying to kill. Which also makes sense.
-
Not sure, tamoxifen is a blocker, it blocks estrogen from binding to estrogen receptors in cancer cells. And AI reduces estrogen in the body. Yes, as a result cells could die of starvation, but I don't know if it slows their growth if they didn't die. All this is too complicated and a lot is still unknown even to the best of minds in the field.
-
@Clairinez, thanks for the comforting words. I'm seeing my MO in next week and will bring up this up in that visit. Guess I'm just not past that feeling of terror when I read something that contradicts my treatment. As many have reminded us, so much is still very unknown.

My MO told me not to take any supplements without consulting her and/or the pharmacist at the clinic, because some might interact with the chemo or the rads. She said this when I asked her about multi vitamins!
I see her again in between surgery and rads. I will ask her about timing then, and probably shouldn't stress too much between now and then.

My Dana Farber onc started mine before rads. She said the evidence that rads and T counter each other is weak.
I have 95% ER+. Because of that, she wanted to get me on TamoxASAP.
-
I started a week after rads. MO was good with it, RO wanted me to start it away but understood my hesitation due to possible SE's. I am also 95% ER+ So far so good

My MO at Johns Hopkins just wrote the script for Tamoxifen after chemo. When I saw my radiation oncologist, also at Hopkins, I asked if I could start Tamoxifen before rads, he said it was fine. He seemed to be very much up to date on the latest & greatest at the time in 2006, because he recommended radiation with 1 to 3 positive nodes, which has since shown to reduce risk of local recurrence in later studies. He stated there was no evidence to indicate taking Tamoxifen while undergoing rads would reduce the efficacy of radiation. He stated the main reason they didn't want to give them at the same time was side effects, and not being able to tell if the side effects were from radiation or Tamoxifen. I believe some studies since then have show there is no major increase in side effects. I tolerated both well, and after radiation, I had my ovaries removed so I could take an aromatase inhibitor, which had shown in studies to be more effective at reducing risk of recurrence.

There is an active study on concurrent arimidex and radiation therapy, I believe it in phase III. Some earlier smaller trials demonstrated definitely no harm from taking AIs during radiation therapy and maybe some benefit:STARS Breast Trial
-
NCCN guidelines for breast cancer (Version 2.2016) currently indicate for adjuvant (post-surgery) treatments:
"Chemotherapy and endocrine therapy used as adjuvant therapy should be given sequentially with endocrine therapy following chemotherapy."
"Available data suggest that sequential or concurrent endocrine therapy with radiation therapy is acceptable."
